Case Manager - Rapid Rehousing

The Rapid Re-Housing Case Manager will provide stabilization supports and services to young adults, aged 18-24 years old that are unstably housed and/or experiencing homelessness in Manchester, NH. The Case Manager will play an active role in assisting program participants to identify and move into, as well as sustain, stable housing. This will require a varied skill set to include, effectively outreaching, and collaborating with potential landlords regarding program goals, in addition to effective intervention and support to young adults as they learn independent living skills.  Reporting to the Program Manager of Runaway and Homeless Youth Programs, the Case Manager is an energetic, creative, and a diligent member of a team that includes a Runaway and Homeless Youth Social Worker, Street Outreach Case Managers, and Transitional Living Program Care Coordinators in Manchester.  Responsibilities include providing comprehensive case management in youth apartments and community settings, creating individual service plans, providing counseling and advocacy, while focusing on positive youth development. 

Candidates must be willing to work flexible hours to build and maintain relationships. The chosen candidate will be comfortable building relationships with clients, landlords and property managers.  Previous experience with Office 365 and Sharepoint desired. The chosen candidate will be required to submit to a criminal background and motor vehicle record check and have a valid driver’s license and auto insurance which meet agency standards. Requirements:

Bachelor’s degree in social work or related human service field required, along with at least one-year experience working with adolescents/young adults preferred. Demonstrated knowledge of and experience working directly with young people through approaches that emphasize resiliency, strength-based youth development, and trauma informed care required. The ideal candidate will have strong multi-tasking skills with a demonstrated ability to manage multiple projects at once with excellent verbal, written and problem-solving abilities.
