We are looking for Community Care Coordinator to join our team right now! Come find your purpose while you make valuable, rewarding impacts in the lives of the individuals we serve.

When you join our dedicated staff you will serve as a “one stop” resource for coordination of care. In this role you will assist in accessing necessary services, working in collaboration with other service providers to maximize use of existing resources.Visits may occur in families’ homes, at the agency, via tele-health or in the community. This role is key in coordinating care delivery mechanisms with client, caregivers, providers and others and working with the Director to assist, develop, implement and maintain pilot models/initiatives. In this role you will also complete reports that include meeting minutes, statistics, care plans, intakes, assessments and systems change mechanisms implemented inter and intra-agency that increases services to at-risk seniors.

Requirements:

Qualifications: Master’s degree required, in Social Work or related field, five (5) years’ experience providing social work services to the respective community and demonstrate a genuine interest and understanding in working with the at-risk seniors.

Compensation: Pay is based on experience and certifications.

Hours: Weekday schedule with occasional after hours or weekend events.
