Intake Nurse – Registered Nurse (RN)

Cornerstone VNA is a non-profit home, health and hospice organization serving Rockingham, Strafford, Belknap and Carroll Counties in New Hampshire and York County in Maine.  The team at the VNA uses the latest technology to provide the most highly skilled nursing, rehabilitative therapies, social work, and support services in their service area.

Certified Specialty programs include Wound, Ostomy & Incontinence care, Diabetes Management and Education, Certified Behavioral Health Nursing, Certified Intravenous Therapy, Chronic Care Management, Palliative Care Consultations, a Maternal Wellness Program and Fall Prevention & Balance Therapies.  Specialty services include a Telehealth Program, an in-home health monitoring system, and Smart Care, an emergency response and communication system for patients and their families providing 24 hour, 7 day a week peace of mind and added sense of security when making the transition to homecare.

Position Description:

The Intake Nurse processes referrals, triages patient and staff phone calls, processes provider orders, lab reports, and other clinical documents.

Educational Requirements:

  • Current NH (or compact state) Registered Nurse (RN) 
  • Two or more years of homecare experience preferred

Other Requirements:

  • Strong computer skills: internet portals, email, using multiple screens and portals at one time
  • Flexibility to cover weekends/holidays/during vacations
  • Strong interpersonal and communication skills
  • Critical thinking, problem solving and great organizational skills a must
  • Independence and initiative
  • Exceptional assessment skills
  • Computer competency, proficiency with medical EMR
  • Clean and valid driver's license along with vehicle insurance are required 

You will function to coordinate the transmittal of information between staff in the field and physician's offices, relay appropriate changes in orders, schedule to appropriate personnel, and work closely with others members of the office team.  You will be available to support additional functions, which occur in the office. Patience and flexibility are essential, in addition the ability to be interrupted then return to an unfinished task. This is a full-time position with an excellent benefit package.
