Options Counselor - Veterans and Caregivers

Partnership for Public Health

The Partnership for Public Health mission is to improve the health and wellbeing of the greater Lakes Region through interorganizational collaboration and community and public health improvement activities.  The goal of public health is to improve the health of individuals, families, and the community through disease prevention, health promotion, and protection from enviornmental threats.

Through a contract with Partnership for Public Health, ServiceLink Resource Center if Belknap County helps individuals access and make connections to long-term services and supports, access family caregiver information and supports, explore options and understand and access Medicare and Medicaid. ServiceLink is designated as New Hampshire's Aging and Disability Resource Center and the NHCarePath Full Service Access Partner providing services for individuals of all ages, income levels and abilities.

 

 


ServiceLink Resource Center of Belknap County is seeking an Options Counselor to work out of the Laconia office. Responsibilities primarily include providing information and referral, needs assessments, care planning, and case management for family caregivers, Military service members, Veterans, and their families. Ideal candidate will also have some working knowledge of public assistance programs and local resources. 

Requirements:

Bachelor’s degree preferred with three years’ experience in areas of aging, disabilities, community health, case management, or other human services related field.
