Case Manager

Case Manager - Integrated Care

Riverbend Community Mental Health, Inc., one of the largest providers of behavioral health services in New Hampshire, is seeking a full-time Integrated Care Case Manager to work in a local primary care practice in our growing integrated care initiative.

As an Integrated Case Manager, you will be involved in coordinating social services and resources for the patients at Concord Family Medicine primary care practice, in Concord, NH. 

  • Bachelor's degree in Social Work, Psychology, Human Services or a related field OR in-depth experience related to the position
  • Valid NH driver’s license, personal vehicle, and auto insurance or access to a reasonable alternative preferred. 
  • Experience working with people who have a history of trauma, mental health disorders, and/or substance use disorders preferred.
  • Experience working with people from diverse ethnic and racial backgrounds preferred.
  • Participate as a member of a multi-disciplinary team including both medical and behavioral health staff and providers
  • Screen and briefly assess patients to identify their psychosocial and resource needs and appropriate services
  • Arrange appropriate services, make referrals to community resources; educate patients and families regarding these resources
  • Assist patients and their families in problem solving potential issues related to the health care system, financial, or social barriers
  • Serve as the system navigator and point of contact for patients and families
  • Assist patients in completing paperwork and releases of information  as needed
  • Develop and maintain relationships with community resource partners
  • Maintain a directory of current information regarding community resources, eligibility requirements
  • Document contacts with patients and track status of referrals to ensure closed loop
