Amoskeag Health, formerly Manchester Community Health Center (MCHC), is a nonprofit 501(c) (3) federally qualified health center offering high-quality, comprehensive, and family-oriented primary health care and support services since 1993. Now known as The Dr. Selma Deitch Center for Children and Teens, Child Health Services (CHS) is the original pediatric practice of MCHC dedicated to improving the health and well-being of at-risk children. In June of 2019, all locations of MCHC consolidated under one name, Amoskeag Health.

Through all its programs, Amoskeag Health serves over 14,579 patients annually across four locations in Manchester.

This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

Primary care, preventive care, outreach and enabling services at Amoskeag Health are supported in part by the Health Resources and Services Administration (HRSA) of the U.S. Department of Health and Human Services (HHS) under the Health Center Cluster grant number H80CS00571. This grant provides approximately 15% of total revenue.

The PREP/SHINE Program Facilitator is responsible for supporting the development, implementation, and evaluation of the Teen Clinic PREP Program facilitating evidence based curriculum targeting at-risk teens in the greater Manchester area. 

The PREP/SHINE Program Facilitator reports to the Director of Social Services and is accountable to the PREP Program Coordinator.

1.Facilitate ongoing PREP/SHINE classes using an evidence based curriculum designed to provide skill building to at-risk teens in the greater Manchester area.
2.Support the development and implementation of an evidence-based pregnancy and STI prevention educational program as part of the Teen Health Clinic program, to include adult preparation skill building including healthy life skills, healthy relationships, employment skills, educational / career planning.
3.Participate in a collaborative with partner agencies working with the same evidence-based curriculum; work with collaborating agencies to schedule and organize training for the community.
4.Support the development and implementation of community outreach programs to promote the program and general pregnancy and STI prevention education with both service providers and potential clients, especially targeting hard-to-reach populations.

Minimum Qualifications
•Bachelor’s Degree in Social Work or related field, or equivalent work experience.
•Spanish or other language skills helpful.
