Through multiple programs we deliver services that allow people to navigate their lives and live as independently as they choose.


Granite State Independent Living (GSIL) is looking for a dynamic and energetic Full Time Service Coordinator (Tuesday-Saturday or Sunday-Thursday). We are a home care and community-based organization dedicated to maximizing independence for adults with physical disabilities and chronic health care needs. This person will provide intake and orientation to new consumers, assuring ongoing individualized support to consumers assigned. Teach and provide skills training to consumers on how to direct their own care and provide training as needed to the care attendants. Develop and adjust consumer’s care plans to ensure personal care needs are being met. Assist in outreach efforts. Serve as community resource on personal care options.

Requirements:

Must have effective communication and teaching skills and a commitment to facilitating and maximizing independence for persons with chronic health care needs. Provide on-call coverage for the personal care program as needed. Must travel in the Seacoast area of NH. (Mileage Reimbursement is provided) This position will be based out of the Dover NH Office.
Education: BA/BS preferred or a minimum of two years’ experience working with persons with disabilities and/or health care needs required.
