Clinical Case Manager

North Country Health Consortium

The North Country Health Consortium (NCHC) is a dynamic and innovative non-profit organization created in 1997 as a vehicle for addressing common issues through collaboration among health and human service providers serving Northern New Hampshire. NCHC is engaged in activities for:

  • solving common problems and facilitating regional solutions
  • creating and facilitating services and programs to improve population health status
  • health professional training, continuing education and management services to encourage sustainability of the health care infrastructure
  • increasing capacity for local public health essential services
  • increasing access to health care for underserved and uninsured residents of Northern New Hampshire

The following Full-Time position is available within NCHC's Substance Use Disorders Clinical Services at the Friendship House location, a comprehensive residential treatment facility that serves individuals with substance use and co-occurring mental health disorders. 

  • Bachelor’s degree in social work, public health or other related field preferred, and experience in client-related work
  • Familiarity with substance use treatment recovery models
  • Ability to develop and maintain healthy and professional boundaries and strong desire to provide support and structure to individuals challenged by a substance use disorder
  • Valid drivers’ license, safe driving record history, reliable transportation, and proof of insurance
  • Willingness to become CPR/First Aid certified and attend required trainings to be eligible for Certified Recovery Support Worker (CRSW) credential within 6 months of hire

• Case manage both high/low-intensity programs and assist with case management of Intensive Outpatient Program, as needed Meet with clients daily to apply for a variety of benefits, navigating state and insurance processes • Assist clients with setting medical/behavioral health appointments, transportation, medication-assisted treatment, and other needs • Teach classes to clients about personal case management• Support residents’ capacity to follow through with objectives to meet their treatment goals • Comply with all federal, state, and local regulations regarding services for substance use, mental health, homelessness, housing including rights of recipients and mandatory reporting
