Hospice/Palliative Care Nurse Practitioner

Position Description:

The Hospice/Palliative Care Nurse Practitioner is accountable to the Hospice/Palliative Care Director for consults and management of PC patients in the community and face-to-face visits as needed for Hospice patients.   Provides medical expertise on pain and symptom management to clinical staff, interacts with attending physicians for collaboration of patient care regarding pain and symptom management issues and update of patient's plan of care, maintains documentation of patient's visits and assigns appropriate billing code that will support the level of service per the regulation. Acts as a liaison to the community in the capacity of educational sessions as requested by the Hospice/Palliative Care Director. Remains current on clinical standards of practice in hospice and palliative care; promotes evidence-based practice. Assists the Hospice/Palliative Director in review of patient referrals and patient's appropriateness for services, identifies home care patients for appropriateness for home care or hospice services and various other duties as assigned by the Hospice/Palliative Care Director.

Requirements:
  • Experience in the area of pain and symptom management
  • Excellent communication skills
  • Ability to practice both independently and within an interdisciplinary team with a focus on patient centered care.
  • Knowledgeable in the Hospice philosophy and regulation preferred
  • Must have Medicare Part B billing capabilities with an active NPI #
  • Proficiency with medical EMR systems
  • Valid driver's license in good standing and vehicle insurance required
  • Ability to provide trusted, compassionate and expert care

Educational Requirements:

  • Graduate of an ANCC or AANP certified program
  • Current NP/ARNP license in New Hampshire and Maine eligible
  • Certification in Hospice and Palliative Care preferred
