Nurse Liaison

Concord Regional VNA

Concord Regional VNA  is a not-for-profit, community-based healthcare provider that provides home care, hospice and community support services to  41 communities in central New Hampshire.  Concord Regional VNA has strong ties with these communities and works collaboratively with Capital Region Healthcare and other providers to provide exceptional care to its patients.  Our mission is to improve the health of the people and communities we serve by managing illness and promoting wellness through all stages of life. Job Duties:

The Nurse Liaison works collaboratively with assigned referral sources to facilitate a smooth transition to wherever they call home with Concord Regional VNA services. The Nurse Liaison will be an ambassador for all Concord Regional VNA services by providing education, in-services, and guidance on how patients can access their home care and hospice benefit. This versatile position will cultivate relationships with a variety of professionals, peers, patients, and caregivers. 

  • Assess patients and review medical records to ensure appropriate services and evaluate clinical needs. 
  • Provide health care and resource education along with the benefits of home care and hospice.
  • Attend discharge or family meetings for current or new patients to appropriately advocate for services/needs and communicate necessary clinical information (documentation, verbally, email).


1. Bachelor's degree in nursing.

2. Previous experience in customer service, sales, or care coordination.


Work Schedule:

Full-time with benefits (40 hours per week); Monday - Friday, 8 a.m. - 5 p.m.
