Description

Position: Triage Nurse

Job Description:

The Triage On Call Registered Nurse assures that communication with each caller will be handled in a timely, professional and tactful manner. When appropriate, the caller’s symptoms will be assessed and triaged using the nursing process and approved guidelines to assist in obtaining the appropriate level of care and/or self-care advice. This position reports to the Home Care Director and Hospice Director. This position’s functions are supervised by the Clinical Manager. This position would be on call Monday through Thursday from 4pm to 8am with an On Call stipend plus hourly wage when seeing patients.

Core Duties & Responsibilities:

  • Uphold and support the mission, objectives, and policies of LRVNA.
  • Respond promptly to each incoming call.
  • Triage patient or caregiver phone calls
  • Manage Home Care and Hospice symptoms.
  • If call warrants a triage, the caller’s stated symptoms will be assessed to determine the appropriate level of care required to safely meet the patient’s medical needs (Emergent, urgent, non-urgent or home visit warranted)
  • Available for in-home visits for patient assessment, treatment and palliate symptoms if warranted.
  • When appropriate, provide home care or hospice instructions using approved guidelines.
  • Complete documentation in MatrixCare in compliance with approved policies and procedures
  • Communicates with multidisciplinary team.
  • Actively participate in new employee orientation, ongoing in-service programs, staff meetings, quality improvement, and periodic performance/protocol evaluations and development
  • Abides by personal and professional standards as outlined in employee handbook.

Qualifications:

  • Graduate of an accredited school of nursing.
  • Bachelor’s degree, with one year of home health experience
  • One to two years of recent acute care experience in an institutional or home care setting
  • Current licensure in State, CPR certification and valid driver’s license
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills per competency checklist.

Physical Requirements

  • Prolonged or considerable walking or standing.
  • Able to lift, position or transfer patients up to 50 lbs.
  • Able to perform CPR until emergency services are on scene.
  • Able to lift supplies and equipment.
  • Considerable reaching, stooping, bending, kneeling +/or crouching.
  • Visual acuity and hearing to perform required nursing skills.

Working Conditions

  • Have an insured vehicle in good working condition.
