Community Bridges is looking for full-time START Coordinators for the START Program in New Hampshire!
We are a non-profit agency founded in 1982 with a mission to advance the integration, growth, and interdependence of people with developmental disabilities within their own communities. Building on the strengths of over 1,100 individuals in the greater Capital Area and their families, we work to identify barriers of access and find creative solutions to develop meaningful community engagement.
The START Coordinator provides case management in the START program. Cases assigned by the NH START Collaborative Team Leader are provided after training has been completed. Continued work in the START program requires ongoing training and certification. Working in conjunction with multiple stakeholders and community partners the Coordinator will ensure collaborative communication with all entities involved with each case. The START Coordinator will provide ongoing development of comprehensive service evaluations, cross system crisis plans, intake/assessments, intervention and outcomes planning.
Qualifications:
Education: Minimum Bachelor's Degree with previous experience in I/DD or mental health. Masters Degree strongly preferred.
Experience: At least two years of experience as a clinician or service coordinator with Intellectual or Developmental Disability/Mental Health population, ability to work with complex systems and interest in the population is essential, and can work independently and stay organized work skills along with superb communication and writing skills are essential.
Reliable transportation, a valid driver's license, and proof of automobile insurance.
Compensation and Benefits
- There is a $1000 sign-on bonus for this position - $500 will be paid at the time of hire and $500 will be paid after 6 months in the role.
*Please note, sign-on bonuses are for new hires only. Re-hires and transfers are not eligible at this time.

Specific Duties and Responsibilities:
- Participate in START Coordinator training and ongoing trainings as required.
- Provide systemic consultation by coordinating resources based on individual needs.
- Participate on all trainings and consultations with the National Center for START Services.
- Prepare agenda and document outcomes for individual mentoring session held with START Center.
- Maintain linkages and relationships with community partners.
- Assist with referrals for consultation and treatment as needed.
- Ensure the coordination of support meetings and crisis plans for individuals served through START.
- Participate in recurring meetings with START leadership and clinical education team (provide case presentations on rotation).
- Provide home visits, visits to day and vocational settings as needed
- Development of comprehensive service evaluations, cross systems crisis plans, intake/assessments, intervention and outcomes plans for respite admissions or inpatient hospital admissions and any other applicable documentation of services provided.
- Maintain active caseload of approximately 30 individuals. (Changes may occur depending on activity level of cases).
- Rotate on-call responsibility for emergency support with other START Coordinators.
Certification Requirement
- The START Coordinator position is offered at an (paid) intern status, and includes a 12 month certification process. Online training is provided through the National Center for START Services, with coaching and clinical training support provided by NH START.
- Demonstration of competency (certification) is achieved through successful completion of specified START instruments (as defined in the START Manual) and presentation of a case to a panel of National Center for START Services reviewers.
- Ongoing employment in the role is contingent upon completion of the certification in a timely manner.

Work Environment and Physical Demands:
- Work Environment- can be occasionally hectic with occasional periods of high stress. Occasional to moderate short deadlines and coverage require flexibility in schedule.
- Physical Demands- must be able to sit and work at a computer. Must be able to bend and reach and lift up to 20 pounds.
Travel:
Travel required within the state frequently. Must have access to personal vehicle during all scheduled work hours, maintain valid driver’s license and appropriate insurance, and uphold an acceptable driving record.
