About the Job

Description

Position Summary: The case manager is responsible for coordinating client services.  This includes  evaluating client needs, prioritizing the level of service coordination, determining appropriate and cost  effective services, tracking expenses and documenting outcomes.  The case manager must also  continuously monitor and evaluate services for quality.  

 Essential Responsibilities:    

• Demonstrate knowledge of NH Regulations (He-M 503, 507, 521, 522, 525, 1001, 1201) and ensure  compliance with regulations   

• System Navigator  

• Person Centered Planning  

• Monitoring and Assuring Individual Outcomes  

• Monitoring and Assuring Health and Safety  

• Conduct and review assessments of client needs  

• Develop a Service Agreement including goals, objectives, methodology and timelines based on  assessments 

• Monitor client services   

• Continuously review services, assessing appropriateness and progress toward goals and/or treatment   

• Ensure service delivery meets client needs in an appropriate setting with positive outcomes   

• Ensure services are provided in a cost-effective manner   

• Request revisions or adjustments in services as appropriate   

• Coordinate RFP process to create a new or different service provider as necessary   

• Prepare and maintain documentation   

• As necessary, assist individuals communicate needs, desires and preferences   

• Develop proactive crisis planning   

• Negotiate service contracts with vendors that demonstrate quality services   

• Demonstrate an understanding of individual budgets   

• Assist in decision making process related to vendor reimbursement issues   

• Research community organizations to reduce costs and increase supports to individuals  

• Participate in Agency committees, teams and work groups    

• This job requires regular in-office attendance and in-person visits to home and community-based  locations based on client needs but at a minimum quarterly.  

• Other duties and responsibilities as assigned by supervisor  

Qualifications:     

Education and Experience   

• Bachelor’s degree preferred, consideration may be given to those with an Associate’s degree and a  minimum of two years of work experience in human services   

• 1 - 2 years of experience working in human services preferred   

• Proficiency in Microsoft Office Suite and database systems   

• Demonstrated ability to work collaboratively with system users at every level in the organization   Updated/Approved JM/DB 5.28.24  Service coordinator family directed services final copy   

• Valid driver’s license and access to a safe operating vehicle during scheduled working hours   

• $100,000/300,000 Automobile Liability Insurance   Skills and Abilities

• Demonstrate a strong emphasis on customer service skills, both to clients and all staff   

• Ability to handle multiple tasks simultaneously while presenting a professional demeanor at all times

• Ability to work independently and as part of an inter-disciplinary team   

• Demonstrate creative, innovative thinking and problem solving

• Demonstrate excellent verbal and written communication skills   

• Demonstrate proficiency in Microsoft Office, including Word, Excel, Outlook, and the ability to  master new software
