PathWays is an Area Agency designated by the State of NH to be the single point of entry to community based supports for individuals with developmental disabilities and their families in Sullivan and lower Grafton counties in NH.
Our misson: PathWays is dedicated to expanding the opportunities that enrich the lives of people with disabilities. Our passion is working together with children, adults and their families to create their own desired future in their own community. We provide our services in a spirit of partnership and respect.
Services we provide:
Family Centered Early Supports and Services. Children aged birth to three, who are at risk of developmental delay, often with significant medical issues receive home-based services with a therapeutic focus with goal of minimizing permanent effects of delay.
Family Support. The family is at the center of this service and defines both their needs and the type of engagement they have with the agency. Services provided include respite, educational advocacy, information and referral, transition from school to adult services and person-centered planning.
In-Home Supports. PathWays provides in-home supports to children and adults who continue to live with their families. These services may focus on personal care, skill acquisition and family defined supports. These supports are tailored to each family with the common goal of keeping the family together.
Service Coordination. This service provides a central person to make sure that an individual has all the services and supports they need in all aspects of their life. This includes engaging in the development of individualized service plans and the monitoring of community-based services.
Outreach. Limited support is provided to facilitate the independent living of individuals in the community through support with money management, grocery shopping, or other life skills.
Vocational Supports. The focus of these supports is identifying employment opportunities, getting a job and providing support to maintain it. This includes working with local businesses to develop work opportunities for individuals with disabilities.
Day Services. This is a broad range of services provided to individuals in their local communities. This may include recreational opportunities, volunteer activities, work experiences, relationship building and personal skill development.
Residential Supports. This is a wide range of services, which may include in-home supports to keep families together, adult foster care, individualized supports and small group arrangements. These supports may range in intensity for several hours a day to 24-hour care and may involve specialized medical or behavioral supports.

What You'll Do:
As a Family Support Coordinator, you'll be the bridge connecting individuals and families to vital resources and community support. Your day-to-day responsibilities will include:
- Service Coordination & Advocacy: Providing personalized service coordination and advocacy, connecting families to state and community resources.
- Empowering Families: Balancing formal and informal resources to maximize family control over services and promoting active involvement in the community.
- Guiding Transitions: Assisting families and individuals in planning the transition from school to adult services, including provider selection and natural supports.
- Legal & Financial Support: Facilitating legal processes like Guardianship, Power of Attorney, and Advance Directives, and assisting with budget administration.
- Documentation & Teamwork: Maintaining meticulous documentation, facilitating planning meetings, and collaborating with a friendly, passionate team.
What We're Looking For:
We are looking for a flexible, creative, and collaborative professional with a non-judgmental approach. The ideal candidate will have a strong background in documentation and a passion for serving diverse families.
Required Qualifications:
- A Bachelor's degree is highly preferred, but extensive case management experience will be considered in lieu of a degree.
- At least 2 years of human services experience, with a minimum of 1 year in family support. Experience with developmental disabilities is a plus.
- Excellent verbal, written, and interpersonal communication skills.
- Strong organizational skills and an intermediate proficiency with MS Office.
- Reliable transportation and a willingness to travel regionally or within the state for home visits and meetings.
- The ability and willingness to learn new programs.
