Critical Time Intervention Coach

$2,000 sign on bonus

Pay Range: $22.00 per hour and up depending upon experience)

Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking an energetic and creative person for our full-time Critical Time Intervention Coach position.

This position will be working in our Emergency Services department providing intensive, short-term (up to 9 months) community-based support services to people experiencing a major life transition. Members of this team will work together to support a person’s successful integration into the community and to meet short term goals. Caseloads are intentionally small to allow for more support for each person.

                                                                                                                                 

Responsibilities:

  • Assist people newly discharged from an inpatient behavioral health setting and identify the supports that are necessary to address their individual needs.
  • Assist clients in connecting with the appropriate community supports and to maintain ongoing connection with identified resources and supports.
  • Provide education about community resources and the importance of community engagement to maintain community tenure.
  • Establish and maintain effective working relationships with various local agencies and programs.
  • Communicate and collaborate with the client’s treatment team.
  • Document all interactions with individuals and community support providers in Riverbend’s electronic health records system.
  • Adhere to the Critical Time Intervention (CTI) model and code of ethics.
  • Participate in the Community of Practice quarterly events and monthly calls with the state and related trainings.

Requirements: 

  • A Bachelor's degree in social work, psychology, or related human services field is required.
  • Experience with behavioral health or social determinates of health (SDoH) systems, including case management, including but not limited to housing, employment, food insecurity and entitlements preferred. Comfort in working with people in the community is required.
  • An understanding on the NH system of care, with experience working between systems is a plus.
  • Knowledge of treatment modalities and interventions (such as service coordination, behavior management, independent living skills training, supportive counseling, motivational interviewing, family education and crisis intervention a plus.
  • A valid driver’s license, a personal vehicle and auto insurance are required.
