About the Job
Greater Nashua Mental Health is looking for a Case Manager to join our Assertive Community Treatment (ACT) team! The Case Manager works within multidisciplinary team, providing services to individuals who have a severe and persistent mental illness and/or co-morbid diagnosis, including a history of substance abuse, active addiction process, and metabolic syndrome, that may impact and or complicate their overall treatment, treatment plan, and prognosis for recovery. Meets with clients within their natural environments, providing appropriate clinical interventions.
Duties & Responsibilities:
- Provides direct client services for a percentage of time per week as appropriate for position and as directed by supervisor.
- Provides case management and Function Support services to consumers including medication monitoring, crisis intervention, family supports, therapeutic behavioral support, etc.
- Develops and coordinates the consumer's Individual Service Plan (ISP) within the Agency and with eternal agencies; communicates, collaborates, and consults with other staff and agencies. Ensures the ISP services are carried out when the consumer allows.
- Serves as the consumer's advocate in making appropriate referrals and obtaining needed services both within GNMH and in the community.
- Assists clients in exploring the potential for future growth opportunities.
- Completes all treatment plans based on consumer history of illness, assessment, and past and current function, results of evaluations, and multi-disciplinary review, by due date listed in EMR. Provides clinical input and formulation of ACT Treatment Plan, which identifies specific problems to be addressed, target goals related to problems, and clear and measurable objectives with clear time frames attached to specific service to be provided.
- Provides necessary contact and treatment after hours and on weekends, as needed or determined by the treatment plan, including afterhours emergency consultation coverage on a rotating basis.
- Assess and evaluates clients in crisis; Provides a clinically appropriate resolution up to and including involuntary hospitalization if necessary.
- Collaborates with the client's representative payees, and guardians, and identified social network for continuity of care, as needed.
- Completes documentation within 3 business days (with first day being date of service/completion).
- Prepares for and attends all meetings as assigned by supervisor; meetings to include daily team meetings, client center collaborations, department meetings, individual supervision, group supervision, etc.
- Prepares for and participates fully ACT fidelity audits and quality service review audits.
- Applies consumer treatment and interventions in alignment with the ACT fidelity model both in service intensity and frequency
- Performs other duties or special projects as required or as assigned.
Qualifications:
- Bachelor’s Degree in Social Work, Psychology, Human Services or related field
- 1 to 2 years of related work experience
- Familiarity with community resources
- Ability to teach effectively
- Demonstrated skill in crisis intervention
- Knowledge and experience in case management services
- Knowledge of and ability to use Microsoft Office products (Word, Outlook, Excel, etc.)
- Strong collaborating skills, and an ability to work effectively both independently and as part of a team
- Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive
- Excellent communication and interpersonal skills
- Ability to multitask, and be flexible with regard to workload and assignments
- Ability to work within an interdisciplinary treatment team
- Ability to advocate for clients both within the team and to community resources
- Ability to problem solve and critically think through problems
- Ability to exercise clinical judgment
- Ability to work flexible work schedule which includes weekends and holiday coverage on a rotating basis
- Must possess a valid driver’s license and automobile with liability insurance in the amount required by agency policy
