Through multiple programs we deliver services that allow people to navigate their lives and live as independently as they choose.


Granite State Independent Living is looking for a Registered Nurse Coordinator. The right person will provide assessment, evaluation, planning, and community support of assigned LTS program services. Teach and provide skills training to care attendants and skills training to consumers on how to direct their own care. Review consumers care plans to ensure attendant care needs are being met and adjust as needed. Assure all paperwork is completed in accordance with GSIL Policy and Procedures. Assist in outreach efforts and serve as community I & R for home care options as needed.

Essential Responsibilities:
• Participate in the assessment and evaluation process for determination of eligibility for participants in attendant/home care programs.
• Consult with individuals about health care concerns and provide information on available options.
• Consult with other staff about individual participants in attendant care programs.
• Provide orientation and skills training to consumers regarding attendant care program polices and procedures.
• Address health care management issues and emergency medical planning.
• Monitor participates' care plan on a bi-monthly basis and more often per regulations/as the consumer's needs change including annual reassessments. Provide ongoing support and training as needed.
• Coordinate services and keep and maintain accurate records for consumers. Assure consumer records are maintained in accordance with GSIL policy and procedures.
• Serve as an information resource for community and staff on personal care options, community supports, and health and disability care related issues.
• Conduct other duties as assigned that will increase options for persons with disabilities to live independently in the community.
• Assist consumers with Nursing Facility Transitions and monitor status on an ongoing basis.
• Coordinate, train, supervise, and schedule care attendants specific to the Home Care program.
• Training /Supervision of assigned Personnel

Other Responsibilities:
• Work a flexible work schedule in accordance with department needs
• Provide on call coverage as needed for the department on a rotating basis
• Participate in general GSIL events and activities.
• Maintain professional knowledge /skills by reviewing professional literature and attending trainings /workshops.
• Support and uphold workplace regulations and safety practices.
• Share newsworthy information appropriately throughout the organization.
• Conduct other duties as assigned that will increase options for persons with disabilities to live independently in their communities.

Qualifications:
• Registered Nurse with a current New Hampshire license and two years of experience working with individuals with long term health care needs. Must have effective communications and teaching skills and a commitment to facilitating and maximizing independence for persons with chronic health care needs. Travel is required. Must have a valid driver's license and a reliable auto with 100/300 liability auto insurance. Must pass a Criminal background, NH BEAS, & NH Motor Vehicle Record check.
