POSITION SUMMARY
The State Service Clinician is responsible for facilitating individual, family, or parent therapy sessions and assessments as determined by and according to program standards.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The essential functions include, but are not limited to the following:
· Providing home-based individual therapy, family therapy, and case management to adolescence and their families.
· Creating and updating treatment plans, skills development plans, progressive independent living plans or any other plan mutually agreed upon by the referring agency such as Juvenile Justice or DCYF.
· Complete clinical documentation as required by state and insurance contracts, referring agencies, program requirements. All clients will be tracked through SYS’ electronic medical records with the expectation that records are kept current. These records include but are not limited to:
- Intake packages and assessments
- Treatment plans
- Progress notes
- Contact logs
- Weekly Reports
- Court Reports
- 30-60-90-day updates
- Discharge summaries
· Collaborate with stakeholders including partner social workers, family networks, referring agencies, therapists, guidance counselors, case workers, and probation officers.
· Attend weekly team meetings and discussion of cases.
· Facilitate monthly team meetings with referral source and the family
· Daily driving/transportation
· Attend court as needed
· Participate in an on-call rotation providing emergency services for clients, according to established on call procedures.
· Perform other duties as assigned
MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
· Current license in the state of employment preferred (LMFT, LICSW, LCMHC) or working to become license eligible
· One year of clinical experience preferred
· Exhibits passion for our clients, values, and organization
· Demonstrates excellent clinical customer service using therapeutic communication skills to build strong client relationships and improve outcomes
· Participates in providing clinically competent care by self/staff/team through education, role modeling, teambuilding, clinical documentation, and quality monitoring
· Uses leadership, team building, negotiation, and conflict resolution skills to build partnerships within and across system
· Engages in self-evaluation process, seeking feedback regarding own practice, from clients, peers, professional colleagues, and others
· A valid driver's license with insured transportation is a requirement of this position.
