Family Promise of Greater Concord is an affiliate of the national Family Promise organization, an interfaith nonprofit organization started in Union County, NJ, committed to helping homeless families nationwide achieve lasting independence. We opened our doors in Concord in the fall of 2015, partnering with the local community, social service agencies, and a large network of amazing faith-diverse volunteers from local surrounding places of worship. Together, we provide shelter, meals, hospitality, and professional wraparound case-management staff to address families' immediate needs and help them move into stable housing.

Family Promise of Greater Concord Seeks Full-Time Shelter Case Manager

About Family Promise

Family Promise of Greater Concord (FPGC) helps families experiencing homelessness to achieve sustainable independence through a community-based response. FPGC offers short-term emergency shelter, food support, case management, and housing navigation services. We help to prevent households from becoming homeless and support program graduates through extended stabilization services.

Case Manager (CM) Job Objective

This organized, compassionate, and motivated person will quickly transition families from homelessness to permanent housing. The CM will also assist families in obtaining outcomes such as securing employment, increasing earning potential, and securing/maintaining housing.

Responsibilities:

  • Lead the intake process for all potential families: conduct intake interviews including talking with referral source(s)/partners, conducting background checks and drug tests; conduct orientation for new families moving into the shelter program.  
  • Assist with family job applications and interview preparation; budgeting/financial coaching; getting families connected to necessary resources (daycare, counseling, educational programs, etc.)
  • Find housing for families by liaising with landlords and getting families ready as needed: debt repair, ensuring families can get utilities turned on in their name, will have furniture, etc.
  • Work with other Case Managers to deliver resources, referrals, and workshops related to employment, financial literacy & budgeting, housing plans, and educational planning.
  • Serve as main contact for host-congregation volunteers and ensure they have necessary family information (work schedule, food allergies, etc.).
  • Develop relationships with partner organizations and maintain database of resources for families.
  • Serve as an advocate for families with other community service agencies.
  • Provide crisis support to families and volunteers as needed.
  • Conduct weekly individual meetings with families.  
  • Additional responsibilities as needed. 

Administrative Responsibilities:

  • Document all case management activities. 
  • Manage on-call phone for families and volunteers.
  • Collect and manage data accurately and efficiently.
  • Maintain case file records for every family, meeting program, funder, and family needs.
  • Provide weekly program updates to Executive Director.
  • Help keep the day center looking presentable.
  • Work with/supervise volunteers and interns as needed.

Additional Responsibilities:                                                                                             

  • Schedule is expected to include at least one evening shift every other month (volunteer coordinator’s meeting).
  • Must have a valid driver’s license and willingness to occasionally use personal vehicle to transport guests if necessary (reimbursed for mileage).
  • Some lifting of materials/donations.
  • Assistance at fundraisers and special events as needed.

Education, Experience, and Skills  

  • The successful candidate is enthusiastic and can display good judgment, take initiative, and help families achieve outcomes.  Must have a can-do, figure-it-out attitude.
  • Four-year degree (BSW, psychology, or related degree) desired.
  • Minimum 3 years’ experience in direct service related to housing/shelter and/or family services.
  • Expected case management skills include trauma-informed care, rapid-rehousing, housing-first, and a strength-based approach.
  • Must be detail-oriented, execute projects in a fast-paced, often pressured environment; able to prioritize, self-manage, work independently, and use good judgment at all times.
  • Must possess a positive attitude, modeling this for families we serve.
  • Must be proficient in computer programs, able to track outcomes, and record case notes.
  • Comfortable working with various constituents: guests we serve, faith community volunteers, corporate and foundation partners, etc.

Salary and Benefits

  • This is a full-time, on-site position without benefits.
  • Salary range is $40,000 - $44,000
  • Ten days PTO.
  • Mileage reimbursement for work-related car travel.

Work Environment & Managing Success:

  • The Case Manager reports to the Executive Director.
  • Occasional lifting of materials up to 25 pounds is necessary.
  • Family Promise is comprised of a small staff and a large pool of volunteers.
