About the Job

Description

The mission of TLC Family Resource Center is to promote the optimal health and development of New Hampshire children and families. The registered nurse, in collaboration with a multi-disciplinary team, works with families to improve health outcomes for pregnant women and children. The nurse also serves as the home visiting coordinator, providing consultation and supervision as needed to family support specialists, medical residents, and collaborating agencies. 

ESSENTIAL DUTIES AND TIME ON TASK EXPECTATIONS 

Direct Family Support (50%) 

  • Works collaboratively with a multidisciplinary team 
  • Makes home visits to families to provide health education to pregnant women and parents 
  • Assesses family strengths and helps families to develop individualized family goals 
  • Conducts depression screening for all enrolled women and makes referrals for mental health counseling as necessary 
  • Educates women regarding family planning and sexuality 
  • Provides tobacco reduction education and support 
  • Screens for alcohol and substance abuse and makes referrals as necessary 
  • Screens for domestic violence and child abuse/neglect and makes referrals as necessary 
  • Connects families to community resources 
  • Acts as liaison between families and medical providers 

Direct Community Support (15%) 

  • Lactation support, in home, center or community 
  • Facilitate drop-in support groups 
  • Facilitate workshops  

Collaboration and Consultation (15%)  

  • Consults with home visitors regarding maternal and child health issues 
  • Consults with collaborating agencies   
  • Community faculty for DHMC, takes pediatric first year interns on home visits  
  • Serves on state and local committees relating to maternal and child health 

Documentation (10%)

  • Complete required electronic documentation and program specific forms thoroughly, accurately and on time using Apricot, QuickBase and other data platforms as required by contract 
  • Follow and adhere to agency policies related to securing confidential information
  • Accurately report time worked using TimeWorksPlus   

 Meetings/Training/Supervision (10%) 

  • Participates in team meetings and individual supervision with RN 
  • Participate in supervision and self-evaluation
  • Participate in required program specific trainings and approved professional development 
  • Meet time on task expectations  


SUPERVISORY RESPONSIBILITIES - None 

 
REQUIREMENTS

  • Registered Nurse Certification 
  • Lactation Consultant Certified (Preferred or willingness to be trained)
  • Maintains current NH RN license 
  • Exceptional customer service skills 
  • Strong analytical and problem-solving skills
