About the Job
The Regional In-Home Care Coordinator is responsible for the day to day management of the Ascentria In-Home Care program in their territory. Responsible for processing referrals for consumer directed personal care services serving the elderly and chronically ill throughout New Hampshire. Provide Intake, orientation and supervision of clients within an assigned territory. Daily supervision of Personal Care Service Providers including HR responsibilities and payroll verification. Duties include marketing the Ascentria In-Home Care program.
Essential Responsibilities
- Responsible for processing referrals for consumer-directed personal care services to include all intake paperwork, care plan development and revision as needed, serving the elderly and chronically ill throughout New Hampshire.
- Responsible for the introduction of potential employees to clients and obtaining all required pre-employment documentation
- Responsible for staffing responsibilities in assigned territory to include interviewing and hiring.
- Work with Training and Compliance Manager to provide training and orientation to clients and Personal Care Service Providers.
- Responsible for the day to day management of the Ascentria In-Home Care program in their territory overseeing client cases and communicating with involved parties.
- Report to Program Manager and Program Director all DOL, Workers Compensation and Medicaid concerns within territory.
- Administrative duties include payroll approval, daily, weekly and monthly reporting, employee HR paperwork and responding to Training and Compliance Manager regarding all compliance. Required documentation must be neat, legible, timely and accurate.
- Responsible for administrative tasks that pertain to clients and Personal Care Service Providers.
- Responsible for the supervision of the PCSP (in conjunction with the consumer), including discipline and enforcing Ascentria policy; provides communication regarding benefits and policy.
- Marketing of the In-Home Care Program.
- Assist with special projects as requested. Perform other related duties incidental to the work described herein to help meet the goals of the department and/or agency.
- Ability to work flexible days and hours.
Qualifications
- Bachelor’s Degree in Social Work or other related human services field preferred. At least 2 years related experience required.
- Supervisory Experience required. Demonstrated ability to serve and care for people in need.
- Ability to handle people and situations with compassion, diplomacy, and tact.
- Excellent interpersonal skills required including written/verbal communication skills.
- Ability to exercise independent judgment and use discretion and an ability to set priorities and to be clear and directive when needed.
- Must possess computer literacy skills including Word and Excel.
- Ability to work cooperatively in a team environment and to problem solve unexpected/unpredictable circumstances calmly. Reliable attendance and the willingness to be flexible.
- Ability to travel throughout NH with own car (mileage reimbursement) to coverage area.
