Granite United Way currently serves as host agency for three (3) out of the thirteen (13) regional public health networks in the state, including the Capital Area Public Health Network, the Carroll County Coalition for Public Health, and the South Central (formerly Greater Derry) Public Health Network. Funded in part by NH DHHS, these networks work to convene, coordinate and facilitate an ongoing network of partners to address regional public health needs.

 

 

POSITION SUMMARY:

This position coordinates public health strategies supported by Granite United Way, with particular emphasis on deliverables related to preventing substance misuse and overdose, enhancing access to suicide prevention and mental health services, and promoting resilience and emotional health for children, youth, and families.

 

This position supports the Capital Area Public Health Network, which serves the following 24 municipalities: Allenstown, Andover, Boscawen, Bow, Bradford, Canterbury, Chichester, Concord, Deering, Dunbarton, Epsom, Henniker, Hillsboro, Hopkinton, Loudon, Northwood, Pembroke, Pittsfield, Salisbury, Warner, Washington, Webster, Weare, Windsor.

  

ESSENTIAL DUTIES AND RESPONSIBILITIES:

 

  • Provide oversight and coordination of the following scopes of services, to meet grant requirements and deliverables, on behalf of the Capital Area Public Health Network including but not limited to:
    • Substance Misuse Prevention (SMP) & Strategic Prevention Framework
    • Overdose Prevention
    • Suicide Prevention
    • Promoting resilience and social-emotional health for children, youth, and families
  • Develop and maintain a regional network of subject matter experts and community stakeholders to support prevention, harm reduction, treatment and recovery, and related health promotion activities.
  • Work collaboratively with partners and key stakeholders to develop strategic plan and work plans. 
  • Coordinate activities within the work plans, with a focus on coalition building, partnering with local school districts, community outreach and family engagement, as well as support and advocacy for other evidence-based prevention strategies. 
  • Ensure that strategies and priorities are data-driven, evidence-based, and culturally appropriate to achieve outcomes. 
  • Organize, facilitate, and provide follow up for coalitions/work groups, such as the Capital Area Leadership Team and Concord Connections Coalition. 
  • Participate in relevant networks and communities of practice around the region and state working on solving issues aligned with current projects.
  • Submit monthly and quarterly evaluation reporting to funding agencies or as requested.
  • Attend all state required trainings, workshops, and monthly meetings. 
  • Participate in current and emerging initiatives that result in increased and/or new service capabilities that address gaps identified in assets and gaps assessments, including collaboration with other providers and systems.
  • Collaborate with State, regional, and local partners to disseminate information to every community in the Capital Area region in order to increase awareness of and access to services.
  • Collect, monitor, and organize relevant data sources and activity outcomes to present to Granite United Way leadership, volunteers, community stakeholders, and funding partners.
  • Work with the team to ensure positive and active communication through presentations and written reports.

 

OTHER DUTIES:

 

  • Ability to represent GUW at social, civic, and business events, some of which are outside of general business hours.
  • Other duties as assigned.

 

EDUCATION, EXPERIENCE, AND SKILLS REQUIRED:

 

  • Bachelor’s degree required; advanced degree preferred
  • Proven experience leading, coordinating and facilitating multi-organization initiatives with diverse stakeholders
  • Strong knowledge of New Hampshire’s systems of care, existing behavioral health and prevention initiatives, and best practices/models with proven effectiveness
  • Ability to manage multiple priorities simultaneously
  • Excellent communication and relationship building skills required: including listening skills, written and oral communication skills, facilitation skills, and presentation skills
  • Experience grant writing and reporting 
  • Ability to contribute effectively as part of a team

 ENVIRONMENT:

 

The venue of this position is a general office environment that is clean, well-lighted, and environmentally comfortable, free of fumes or airborne particles, and only minimally exposes employees to office solvents limited to those relative to copier equipment.

 

This position requires:

  • Minimal requirement to reach at or above shoulder level; occasional reaching below shoulder level required
  • Ability to travel throughout the territory required
  • Ability to work a flexible schedule, which may include night and weekends
  • Ability to tolerate prolonged sitting and/or standing and to lift to approximately 25 pounds to waist high level whenever necessary.
  • Mobility includes regular sitting, some standing and walking.
