Summary

The Physical Therapist (PT) is a key part of our multi-disciplinary clinical team. Our Physical Therapists assess and treat patients in the community, delivering care directly in the patient’s home. Work with a wide variety of health issues such as post operative care for joint replacement or fracture, chronic conditions such as COPD or CHF, home safety and fall risk reduction, to name but a few. Working with these diverse patient types, PTs are often engaged in developing and instructing patients and their caregivers in programs to improve safety, strength, balance, mobility, range of motion and coordination. Care is individualized to the needs and goals of each patient.

Responsibilities: Reasonable accommodations may be enable individuals with disabilities to perform the essential functions.

  1. Interprets philosophy & corporate objectives related to home care to patients & families, referral sources & community groups.
  2. Performs all assessments, treatments and documentation per agency policies and standards which include State and Federal regulations, and APTA Code of Ethics.
  3. Develops, implements, evaluates and modifies clinical services to patients under a physician’s plan of treatment.
  4. Assesses patient’s rehabilitation needs by performing patient tests, measurements & evaluations such as range-of-motion, sensory & manual-muscle tests, gait & functional analysis and body parts measurements.
  5.  Utilizes an Evidenced-Based Management approach to improve home care patient outcomes.
  6.  Documents interdisciplinary communications.
  7.  Instructs patients & family members regarding rehabilitative therapy as part of the plan of care resulting in the patient/family assuming increased responsibility for care when appropriate.
  8. Assists in referring patients to other community resources as needed.
  9. Maintains and enhances current level of knowledge relative to professional practice, as well as continuing education requirements necessary for licensure and certification.
  10.  Renders patient care with a high degree of service excellence, which will be measured by family satisfaction surveys.
  11. Develops, implements, evaluates and modifies clinical services to patients under a physician’s plan of treatment.

Qualifications

  • CPR Certified within 6 months of hire
  • State License: Current N.H. State Registration as a Physical Therapist (PT) and in good standing.
  • Access to operable motor vehicle and a valid NH driver’s license with acceptable record of safe driving practices.
  • Ability to communicate effectively in English, both verbally and in writing.
  • Must be willing to participate in a weekend rotation.
  • Must be able to meet the physical demand requirements of the position.
  • Must have reliable transportation and be able to supply proof of car insurance in the amount of $100,000/$300,000 coverage.
  • Ability to pay close attention to detail and follow verbal and written instructions.
  • Ability to evolve practice patterns to remain current with best evidence methods and the changing healthcare environment.
  • Ability to assimilate and interpret new knowledge and/or regulations.
  • Excellent interpersonal communication skills and organizational skills required.
  • Computer skills strongly preferred.
