Hospice Help Foundation (HHF) is a 501(c)3 nonprofit organization located at Carey Cottage in Portsmouth, New Hampshire. HHF’s mission is to improve the comfort and quality of life for hospice patients and their families by providing urgent financial relief. We partner with hospices throughout New Hampshire, Southern Maine, and beyond in order to provide an extra layer of support to impoverished hospice patients by funding essential needs that they would otherwise not be able to afford, such as rent and heating, warm meals and clothing, transportation costs to stay connected to loved ones, glasses and hearing aid repairs, and modest last wishes. HHF also works with local organizations to educate community members about end of life options.

Hospice Help Foundation (HHF) is a nonprofit committed to providing critical financial support to hospice patients struggling with hardship at the end of life. Founded in 2008 on the New Hampshire Seacoast, HHF was established as a 501-c3 in 2015. Since then, HHF has worked with more than 70 hospices and hundreds of hospice staff in New Hampshire, Maine, and other US states to allocate over $500,000 in financial relief to more than 1,000 hospice patients of all ages and backgrounds. 

The Patient Support Coordinator plays a pivotal role in processing patient applications for financial assistance, collaborating with social workers and hospice staff, and sharing patient stories to inspire donors. This is a gratifying opportunity to make a difference in the lives of individuals and families during their most vulnerable moments. We aim to fill approved patient requests within 24-48 hours of receiving completed applications because, for hospice patients, every day is precious.

Key Responsibilities

1. Application Processing (75%)

  • Review and process patient applications for financial support.
  • Ensure applications include relevant details, such as household income and invoices/quotes for requested services.
  • Coordinate with applicants to gather missing or clarifying information, if needed.
  • Encrypt and securely share applications with HHF’s review committee, ensuring timely feedback.
  • Communicate decisions to applicants, liaising with HHF staff to issue payments.

2. Hospice Partner Support (15%)

  • Build and maintain strong relationships with social workers and hospice staff to streamline the application process.
  • Provide guidance and support to hospice staff as they submit applications on behalf of patients.

3. Story Collection (10%)

  • Collaborate with HHF staff to gather impactful patient stories that highlight the importance of our work for donor outreach.

Qualifications

  • Passion for hospice and end-of-life care; prior experience with hospice is highly desirable.
  • Background in social work or a related field is preferred.
  • Excellent interpersonal and communication skills (speaking, writing, and active listening).
  • Experience building and maintaining partnerships across organizations.
  • Strong time management, prioritization, and multitasking skills.
  • Attention to detail and ability to work independently.
  • Empathy, flexibility, and a commitment to HHF’s mission.
  • Proficiency in Google Suite, Excel, and secure data handling practices.
