Grafton County Senior Citizens Council ( GCSCC) is a nonprofit , charitable organization. Our purpose is to develop , strengthen, and provide programs and services that support the health, dignity and independence of older adults and adults with disabilities living in our communities.
About the Job
As a member of the ServiceLink of Grafton County team, the Options Counselor conducts individual needs assessments to determine appropriate referrals for long-term care needs. Knowledge of the principle and methods of person-centered services. Provide information, referral, and assistance. Certification is required within one year of hire. BA degree in Human Services or related field, or associate’s degree with equivalent experience. Minimum 3 years social work or case management experience. Experience working with older adults and/ or adults with disabilities. Working knowledge of computers and human services. Fully benefitted position.
