Greater Nashua Mental Health is looking for a full-time Nurse to join our ACT Team. The Nurse provides comprehensive nursing support to clients on the Assertive Community Treatment (ACT) team. Supervises the medication administration of all ACT clients, which includes verifying that medications given reflect the most recent orders, packaging medications for delivery by ACT team staff, providing medication management at the Agency for a select number of clients, scheduling and delivery of long acting injectable medications, and maintaining accurate records of all related activities. Provides medication education to clients, families and staff.

Duties & Responsibilities: 

  • Provides functional support services through medication management/education daily and as scheduled for ACT team clients and document accordingly.
  • Obtains and packages medications for delivery to clients both at the Agency and in community. Verifies the accuracy of medication regimen. Maintains timely records of administration. Coordinates the delivery of medications daily. 
  • Conducts proper nurse assessment, schedules and delivers long acting injectable medication, provides patients with education on medications, and maintains timely and accurate documentation of interactions related to patient care, with a 50% productivity expectation.  Collects specimens for laboratory testing and enters orders into online systems.
  • Completes all documentation within 3 business days (with first day being date of service/completion).
  • Reconciles 95% of all prescribed medications at time of client's hospital discharge.
  • Ensures that 90% of refill requests are processed 2 business days.
  • Conducts outreach to outside providers, both medical and psychiatric, on behalf of clients. Attends outside appointments as necessary.
  • Attends team meetings in order to best provide integrated care to clients.
  • Interacts with pharmacies and insurance companies to ensure prompt delivery of needed medications.
  • Conducts interim nursing assessments and medication reconciliation as requested by medical staff, following Just in Time scheduling model.
  • Provides cross coverage of injections for CSS clients.
  • Performs other duties or special projects as required or as assigned. 

Qualifications:   

  • Bachelor’s Degree in Nursing
  • 1-2 years of related work experience
  • NH Registered Nursing License required
  • Knowledge of psychotropic medications
  • Ability to safely deliver intramuscular injections and ability to educate clients and staff on medication
  • Knowledge of relevant nursing principles and procedures
  • Experience using electronic medical records (EMR) systems
  • Demonstrated skill in crisis intervention
  • Strong collaborating skills, and an ability to work effectively both independently and as part of a team
  • Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive
  • Excellent communication and interpersonal skills
  • Ability to multitask, and be flexible with regard to workload and assignments
  • Ability to exercise judgment 
  • Knowledge of Microsoft Office products (Email, Word, EXCEL)
