Lakes Region Mental Health Center (LRMHC) is the community mental health center serving Belknap and southern Grafton Counties. We provide services to 24 surrounding towns, and nearly 4,000 patients each year. A non-profit, private corporation founded in 1966, Lakes Region Mental Health Center is governed by a volunteer Board of Directors dedicated to our mission.  We provide integrated mental and physical health care to children, adults, elders and their families who are living with – and recovering from- mental illness and/or emotional distress. Our dedicated professionals provide the highest quality of healthcare and collaborate with many other organizations in the region to improve access to care and meet the mental health needs of the community. $2,000 Sign On Bonus

The Neurocognitive Case Manager (CM) is responsible for coordinating and assuring the continuity of services for each patient on their caseload.  They provide support with a recovery focus to adult patients with cognitive impairments.  These patients have a primary mental health diagnosis and also are older adults with dementia, adults with intellectual disabilities, and/or adults with traumatic brain injury.  The CM assists patients in discovering and maintaining a valued role in the community.  CM’s work with each patient, and their families and/or support system to coordinate care planning, complete a comprehensive assessment of needs, develop a Case Management Plan and monitor the effectiveness of it, and ensure resource linkage, referral and advocacy.  Case Managers engage families and natural supports to participate with the patient in their recovery process.

Qualifications: Bachelor’s degree in a Human Services Field preferred but not required. One to two years of experience with emphasis in mental health preferred, or lived experience.
