About the Job

Description

Assist with the growth of student transition services for at-risk youth and students with disabilities. This will include the supervision of transition staff, and coordination and management of working relationships with schools and other community partners. This position will work closely with the Director of Education Services to lead and manage all statewide student transition efforts.

This position will be required to assist the Senior Director of Educational Services with implementation and coverage of the IMPACCT Program classrooms throughout the entire state, regardless of location. All responsibilities listed within this description shall be applied to all locations within the state.

Responsibilities

· Supervise Transition staff in assigned regions of the state.

· Build and manage relationships with, schools and community partners that are essential for program success.

· Work with the Director of Education Services to oversee and implement transition services statewide.

· Establish and maintain positive relationships with school districts in assigned regions and community partners to ensure student success.

· Work with the Director of Educational Services to oversee and implement transition services statewide.

· Train new transition staff in the GSIL service model for at-risk students with disabilities.

· Present IMPACCT program model to schools, and community partners.

· Oversee staff providing outreach to youth who have dropped out of school and need to reengage in the educational system and academic recovery activities. Recruit potential students for services using youth, parents, school staff and outreach specialists, medical professionals, disability organizations and other service providers as referral agents.

· Coordinate all service activities with Vocational Rehabilitation NH (VRNH) and High Schools, incorporating community supports as appropriate to foster interagency collaboration using the guidance of a self-efficacy model of service delivery.

· Under the direction of the Director of Educational Services, design and implement workshops/services for students and parents with role models (mentors) as speakers sharing their life experiences.

· Use the person-centered approach and support to guide service decisions.

· Provide support and services to students with disabilities not enrolled but identified by the school as being at risk of failing or dropping out.

· With the student as the informant, develop, plan, and implement programming that focuses on increasing the ability of youth to live life independently.

· Under the direction of the Director of Educational Services, plan and conduct presentations on youth disability topics for the general community, service providers, community organizations, consumer groups, etc.

· Conduct intakes; assess student needs; develop Independent Living Plans with students which will enable them to increase self-reliance and to explore life and education/career choices.

· Maintain high quality records of student and community contacts through the GSIL documentation system.

· Supervise staff in their role of helping students identify and use community supports to address their needs in terms of non-academic needs in areas such as Independent Living (IL) skill development and greater personal and economic independence.

· Conduct other duties as assigned that will increase options for persons with disabilities to live independently in their communities.

· Provide training room coverage in IMPACCT program locations in the absence of staff.

Other Responsibilities

 Maintain professional knowledge /skills by reviewing professional literature and attending assigned trainings /workshops.

  • Support and uphold workplace regulations and safety practices.
  • Other duties as assigned which will increase options for persons with disabilities to live independently in their communities.

Qualifications

Experience working with at-risk adolescents and young adults with disabilities. Knowledge of issues related to independence for youth with disabilities. Strong interpersonal skills and ability to communicate effectively with adolescents, parents, staff, and service providers. A strong commitment to increase the capacity of youth with disabilities to assume control and responsibilities of their lives. A bachelor’s degree in education, counseling or another related field, and a master’s degree is preferred. Reliable personal transportation and driving is required. Ability to drive a 13-passenger vehicle. Ability to travel to IMPACCT program locations as needed.

Physical/Mental Demands

While performing the duties of this position, the individual is regularly required to sit, use a computer, a telephone, and other office equipment and to move throughout the building and attend outside meetings. Employee may occasionally need to exert up to 10 pounds of force to lift carry, push or otherwise move objects (e.g. files or presentation materials). This position requires the ability to express or exchange ideas and to comprehend and present detailed and critical information including but not limited to financial information. Exceptional judgment and creativity are regularly required to evaluate results and make decisions.
