About the Organization

The Partnership for Public Health, Inc. (the Partnership) is a non-profit agency with a mission to improve the health and well-being of the region through inter-organizational and public health improvement activities.  Based in Laconia, NH, the Partnership is one of the State’s 13 regional public health networks and serves as the fiscal lead for ServiceLink in Belknap and Carroll counties. The Agency has a staff of 20 and provides community health education, substance misuse prevention, suicide prevention, and emergency preparedness programs across the Winnipesaukee Public Health Region. 

This position is within a public health organization that follows CDC guidelines and recommendations.

Description

GENERAL POSITION DESCRIPTION:

The Human Services Manager is responsible for management of subcontracts, public education, public awareness, community and provider relations, program review and quality oversight for all human service programs at the organization, including Aging and Disability Resource Center (ServiceLink) and WellnessLink programs. The Human Services Manager is accountable to the Deputy Director, Partnership for Public Health and collaborates closely with the Aging and Disability Resource Center (ServiceLink) Administrator at the Department of Health and Human Services, Bureau of Elderly and Adult Services.  Providing un-biased health information and education to promote healthy aging, community outreach and evidence-informed education. 

 

KEY RESPONSIBILITIES:

  • Develop and maintain and understanding of Partnership for Public Health programs, services, and functions across all subject matter domains to facilitate efficient access to public health services in the greater Lakes Region;
  • Provide leadership and oversight of Aging and Disability Resouce Center programs in Belknap and Carroll Counties and the WellnessLink Program to include short and long-term goals to support agency and program missions;
  • Directs and evaluates program operations activities such as: logistical operations, performance measurement and reporting, subcontract management, management of public education and outreach, oversight of volunteer program, community provider relationship development, program review, continuous quality improvement;
  • Manages a team process within the programs to foster a cooperative working environment between program staff, on-site team members and designated community long-term services and supports providers;
  • Oversight and accountability of all day-to-day processing of Aging and Disability Resource Center sites programs evaluations including operational reports, quality reports, State and Federal reports to NH Department of Health and Human Services (DHHS), Bureau of Elderly and Adult Services (BEAS) by designated time frames;
  • Works with the DHHS/BEAS staff including, but not limited to: Aging and Disability Resource Center Administrator, the NH Family Caregiver Support Program Director, State Health Insurance Program/Senior Medicare Patrol Director, Adult Protective Services, and Bureau of Family Assistance to ensure that appropriate staff establish and maintain effective working relationships and coordination of the Aging and Disability Resource Center model;
  • Assists with the development, direction and evaluation of fund-raising activities including: acquisition, corporate support, special events, grants and contracts;
  • Develops, implements, and directs public relations and communication strategies that increase public awareness and utilization of programs and services provided by Aging and Disability Resource Center and WellnessLink;
  • Assists with fiscal operations including budget development and authorization of expenditures according to the contracted budget agreement with DHHS, the board of Directors, and/or Fiscal agency. Participates in monitoring the program’s financial health and makes appropriate recommendations to the Deputy Director, Executive Director, Director of Finance & Operations
  • Networks with community groups to strengthen and expand the Aging and Disability Resource Center and WellnessLink service networks (faith communities, ethnic/cultural groups, health and human service providers, business, education and neighborhood groups) as well as supports activities that strengthen program recognition at the local, regional, state, and federal level; and
  • Monitors trends, legislation, and regulations that are relevant to the program’s mission and makes program recommendations to PPH leadership team and Board of Directors.
  • Familiarity with Cultural and Linguistic Appropriate Services Standards (CLAS) to address regional diversity and promote health equity.
  • Performs other duties as assigned by Deputy Director or Executive Director 

 

EDUCATION, EXPERIENCE, AND SKILLS REQUIRED:

  1. Bachelor’s degree with 2 years’ experience in supervision preferred;
  2. Obtain certification as Alliance of Information Referral Specialist (AIRS) and Person-Centered Options Counseling;
  3. Experience in case management, social work, or other related human services field and a passion for supporting older adults, people with disabilities, caregivers, Veterans, and their families;
  4. Working knowledge of local, state, national resources, public health, and human service delivery system;
  5. Ability to translate broad goals into achievable steps;
  6. Ability to foster a team process encompassing resource center staff and designated members from outside agencies;
  7. Ability to develop and oversee financial functions of the program;
  8. Ability to plan, coach, train, organize, direct and evaluate the work of others (including hiring, terminating, and supervising office staff);
  9. Ability to communicate well orally and in writing;
  10. Ability to collaborate among all community, regional, state, and federal groups; and
  11. Good interpersonal and computer skills, openness and flexibility in working with diverse groups, enthusiasm for working collaboratively on community issues.
