In our Community-Based Services Program, we are currently seeking a Social Worker/Community Care Coordinator to join our team.  Easterseals is looking for an individual who is passionate about working with mental health, substance use, and medically involved populations.  Within in this role, we receive referrals through Anthem Health Care; who have identified at-risk clients.  Identified clients have spent excessive amounts of time using emergency services and the assigned Community Care Coordinator connects to offer and support with services.  This role requires excellent communication skills, be open to diverse, strong time management and organizational skills,  and have the ability to be person-centered with providing services.

This is a 40-hour a week exempt position, strongly focused out of the community.  This role also offers flexibility and independence with scheduling.  
 

Responsibilities:

Responsibilities include, but are not limited to: 

  • Assessing the needs of at-risk populations
  • Coordinating care delivery mechanisms with the client, focusing on the social determinants impacting health care
  • Completing daily notes and assessments in our electronic medical record
  • Completing monthly outcomes and reports
  • Participation in relevant meetings and/or conferences
  • Educating service providers about specific issues that are impacting the client

Qualifications:

  • Masters in Social Work or related fields OR enrolled in a MA program– we even provide supervision towards licensure!
  • 5 years of experience providing social work services
  • LICSW independent license preferred
  • Strong collaboration and communication skills are a must
  • Valid drivers license and insured vehicle

Hours: This is a salaried, 40 hours/week, full-time position. 

Salary: 50-65,000/year
