Who We Are
The New Hampshire Harm Reduction Coalition (NHHRC) is committed to supporting efforts to reduce the negative consequences of substance use in NH. This includes the training, education and capacity building of all professional and lay people who come into frequent contact with people who use drugs, to reduce stigma and discrimination, improve access to care and support, and other efforts to reduce the negative consequences of substance use throughout the state and beyond. NHHRC sponsors and supports Syringe Service Programs throughout New Hampshire, also working to enhance the application of harm reduction principles in evidence-based practices across all health and social services sectors.

About the Job
The Care Coordination Support staff member is responsible for identifying, engaging, and building supportive and strengths-based relationships with participants, or potential participants, of the Syringe Services Programs (SSPs) of NHHRC. The Care Coordination Support staff member will facilitate referrals, navigation, and linkage to services and supports to meet the self-identified needs of program participants. Care Coordination Support staff will work closely with the Care Coordinators who operate SSPs in Concord, Manchester, and Strafford County to identify support needs. The primary sources of support that may be needed are in person attendance at busy SSP events, supply organization and kit making, and data entry. This position is part-time between 10-20 hours per week.
Care Coordination Support Staff General Responsibilities:
- Regularly attend SSP outreach services in assigned geographical area, as needed.
- Work closely with SSP volunteers to integrate mutually supportive workflows and ensure that supply kits are assembled weekly.
- Assist with participant data entry as needed.
- Assist with updating, printing and distribution of handouts.
- Distribute materials as appropriate - fit packs, wound care kits, hygiene kits, etc.
Additional Responsibilities:
- Adhere to all organizational policies, practices and procedures.
- Participate in collaborative meetings, as directed.
- Contribute to continued program development of policies and procedures that align with the mission and values of NHHRC and SSPs.
- Ensures that all paperwork and documentation is completed.
Skills and Experience:
Minimum:
- 1+ years documented work experience compassionately supporting adults with mental illness and/or substance use disorder.
- Demonstration of excellent interpersonal skills with vulnerable populations.
- Demonstration of excellent communication, organizational, planning and time management skills.
- Demonstration of proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel) with the ability to learn new applications; you do not need to be an expert.
Preferred:
- Knowledge of community health networks in NH.
- Knowledge of local and regional social support agencies and respective social service program applications (housing/nutrition/Medicaid/food stamps/SSI&SSDI/fuel assistance and others).
- Networking connections/relationships with regional social service agencies.
Must be willing to work in a position which is mostly community based and provide services in community locations. Must be willing to work flexible hours to meet position needs. Must have reliable transportation and maintain a valid NH driver’s license. The use of a personal vehicle with mileage reimbursement is required for this position.
