Cornerstone VNA

Cornerstone VNA, a nonprofit home health and hospice organization, was founded in 1913 and proudly serves 38 communities in New Hampshire and Maine. Our mission is to promote the optimum level of well-being, independence, and dignity of those living in the community by providing trusted, compassionate, and expert health care.

Cornerstone VNA places great value on transforming the home health care experience and is proud to provide care for all ages, from birth through end of life.  We are committed to the philosophy that the needs of each patient are best served by the thoughtful integration of specialized programs and services delivered by a multidisciplinary team of professionals.  Currently, our team provides skilled nursing, rehabilitative therapies, social work, and volunteer support through five distinct programs: Home Care, Hospice Care, Palliative Care, Life Care-Private Duty, and Community Care. Offering a wide range of specialized services, such as wound and ostomy care, IV therapy, diabetes education, chronic care management, and behavioral health, Cornerstone VNA is known for providing advanced care for patients with complex needs and chronic illnesses.  Cornerstone VNA also offers a sophisticated Telehealth Program, a special in-home health monitoring system with video capabilities, which has proven to improve the quality of life and reduce hospital readmissions for high-risk patients.  With our deep commitment to providing high-quality care, Cornerstone VNA continues to be recognized for excellence, and many of our clinicians have been recognized with professional awards for their outstanding achievements.

As a nonprofit organization, Cornerstone VNA is also proud to offer complimentary programs and services that have a positive impact on our local communities. Through our Community Care program, we provide free monthly wellness clinics throughout our service area as well as an Educational Series, which is open to community members, businesses and health professionals.  We also provide support to local caregivers through our annual Caregivers Connect event and through our monthly Caregiver Cafés.  Additional community programs include fall preventions classes and grief support groups.
