Cornerstone VNA

CornerStone VNA is a not-for-profit agency since 1913, committed to bringing home, hospice and community healthcare and supportive services to people of all ages so that families can stay together at home, even when facing the challenges of aging, recuperation from surgery, dealing with a chronic or life-threatening illness or dying.

Our home care program provides services 24 hrs a day/7 days a week to meet the intermittent skilled health care needs of homebound individuals that need skilled nursing visits, rehabilitation visits (physical, occupational or speech), consultations with medical social workers, support services of a licensed nursing assistant, a personal care service provider, or a homemaker to assist with the activities of shopping, laundry, light housework and errands. Our target population is patients of all ages in need of skilled, intermittent home health care.

We also provide services in our Adult Day Center which promotes quality of life and independence for adults through health, social, and supportive services.

The Center not only supports families who choose to care for a loved one at home, it also helps adults in the community maintain optimum health and functional independence. Participants are part of a learning and sharing environment through interaction with others.

The Adult Health Program is designed as a community outreach, illness prevention and health promotion program. Many times these clinics identify problems in the early stages when cost effective intervention can prevent serious illness.

Our focus for 2011 is to restart our Hospice Program in response to a change in the healthcare environment. CornerStone VNA’s goal in providing hospice is to keep as many of our patients in their homes where they prefer to be, with good pain control and an excellent support system in their last days of life. We will be able to offer to our patients at the end of life the ability to continue care through CornerStone VNA, with staff they are familiar with and have come to care about. Providing care under our hospice program will complete our circle of caring from perinatal to end-of life care.

  • Northeast Healthcare Quality Foundation recognized the agency for its active participation in quality improvement activities including: The REACH collaborative- intensive project focused on reducing acute care hospitalizations.
  • CornerStone VNA was recognized by OCS (a national company that reviews and benchmarks agencies from around the country on their OASIS documentation). We received their highest award: the Over-all Vision Award for improved outcomes for patients in 2009.
  • We have been recognized and named again in the top 25% nationally for the third consecutive year by the leaders in patient satisfaction (Fazzi) as an agency of distinction for demonstrating superior performance in patient satisfaction.
