Purpose/General Responsibilities:

In accordance with the vision, mission and values of GSIL, the Attendant Care Worker II is responsible for providing quality assistance to people with disabilities and chronic medical conditions in their own homes with the activities of daily living (such as , but not limited to, eating, dressing, bathing, homemaking and toileting) which may be under the direction of a consumer, to perform the tasks identified in the care plan as assigned. The Attendant Care Worker II will follow the consumer's plan of care. The assistance is provided in the consumer's home and travel is required.

Specific (Essential) Responsibilities:
•Direct Care Responsibilities:

Provide direct care services to consumer who may possess an array of disabilities, physical, mental health, behavioral; in accordance with their care plan to include, but not all inclusive, of the following responsibilities:
•Assist consumers with personal care activities, which may include: bathing, dressing, grooming, as well as bowel and bladder.
•Assist consumers with housekeeping activities, which may include: sweeping, mopping, dusting and general.
•Assist consumers with laundry activities, which may include: washing, drying, folding, and putting away laundry, changing linens, and ironing clothes.
•.Assist consumer with transfers, such as Hoyer lifts and ceiling lifts.
•Assist consumer with meals, which may include: cooking, setting up, assisting with intake of food/hydration, storing meals, and washing.
•Assist consumer with shopping, which may include: grocery shopping, and personal shopping.
•Provide companionship services to the consumer.
•Provide medication assistance consistent with program rules.
•Provide wellness support consistent with consumer care plan.

Other Responsibilities*:***
•Provide on-call coverage, which is defined as the ability to cover open shifts, as needed in accordance with designated work schedule.
•Provide documentation (such as but limited to: task data, change of status, etc.) in accordance with GSIL policies/ procedures for an array of service models / programs.
•Must possess reliable transportation, meeting GSIL insurance and safe driving record requirements.
•Attend mandatory orientations and yearly mandatory in-services.
•Attend any additional training required to provide a safe, healthy environment for you and consumers assigned.
•Must be willing to work with a variety of consumers as assigned.
•Maintain an upbeat and professional attitude.
•Support and uphold workplace regulations and safety practices.

Qualifications and Requirements:
•Must be at least 18 years of age.
•Preferred, experience in an array of transfer devices.
•Preferred, experience in an array of durable medical equipment.
•Preferred, experience and comfort in supporting persons with physical, cognitive, and/or mental disabilities.
•Preferred, experience with personal care.
•Possess effective interpersonal and communication skills.
•Must be adaptable and flexible to meet the needs of consumers.
•Must have reliable transportation and must be willing to travel as needed to report to work.
•Must have a commitment to maintain confidentiality.
•Must be dependable, responsible, patient and punctual.
•Must be able to maintain professional and appropriate relationships with consumers.
•Must be their own legal guardian.
•Must be able to perform the duties of the job safely.
•Sensitivity to the needs of persons who are significantly physically disabled.
•Willingness to be oriented and trained by an employer with disabilities.
•Flexibility in terms of tasks performed.
•Meet the training and experience requirements of the program (i.e.: pass the HC & ACE training)

Physical/Mental demands:**

While performing the duties of this position, the individual, during an average work day, may be required to: lift, push/pull objects including but not limited to household items, individuals, to implement care plans and daily living activities. Activities include: sitting, standing, walking, driving, bending, stooping, climb stairs, kneel, balance, squat, grasp, crouch, crawl, repetitive hand movements, exercise; muscle strengthen when part of consumer care plan, and reaching. You may also be asked to lift or support a weight of 40 pounds in the process of assistance individuals during transfers and using transfer devices.
