Laconia Housing Authority

Laconia Housing, located in the Lakes Region of central New Hampshire, is uniquely positioned as a public housing authority owning and managing 311 units of affordable housing and operating as a licensed home health care agency able to provide supportive services to our residents including meals, personal care, laundry, housekeeping, and limited nursing. Currently, Laconia Housing serves 60 primarily senior households under either HUD’s Congregate Services Housing Program or under the Medicaid waiver Choices for Independence (CFI) program. Laconia Housing is developing a 16 unit project designed for seniors requiring extensive services to remain living independently. In addition to filling the new project, the Supportive Services Director will be responsible for marketing and expanding the current CFI population by up to 50%.


Laconia Housing is a licensed Home Health Care Provider (HHCP) with a heart and delivering personal care services for over 20 years to our residents in the city of lakes, Laconia NH.

We are seeking an energetic, caring health care provider to manage and grow our services under Medicaid Choices for Independence (CFI) program as well as Congregate Housing Services Program.  Our primary patient base is geriatric.

Our programs for patient services are offered Monday – Friday from 8 AM – 4 PM.


Candidate will have 5 years of supervisory experience, 5 – 7 years of experience in the delivery of health care services, current State of NH RN License, prior experience working with the elderly or disabled persons.  Previous knowledge of State Home Health Care Provider and Choice for Independence Program a plus.

Home Health Care Director shall oversee the daily operation and administration of the Supportive Services Department as well as participate in long term strategic planning for supportive services implementation, working towards agency wide strategic goals.

  • Ability to discuss care plans with providers and case managers.
  • Knowledge of Microsoft Windows, Office Suite Products and recommend clinical record implementation.

  • Accuracy and attention to detail and highly organized.
  • Flexibility in approach to a constantly changing work environment.
  • Must be a resident of NH and able to pass a state of NH criminal check and hold a valid NH driver’s license.
  • Lift and carry 50lbs.
  • To maintain composure under adverse conditions.
  • Pay close attention to details.

All job activities and interactions with staff, residents (and their families), clients, government officials, providers, vendors and community members shall be in accordance with the Laconia Housing values of: Integrity, Compassion Professionalism, and Respect.

  • Develop training programs including Wellness and in-service programs for clients, residence and staff.
  • Monitor and recommend implementation of regulation changes at both the state and federal levels, specifically as it relates to Medicaid program and the Choices for Independence waiver.
  • Develop and implement a record keeping system to maintain Laconia Housing’s status as a licensed Home Health Care Agency.
  • Maintain confidentiality protocols to keep Laconia Housing compliant with HIPPA and other federal and state regulations.
  • Oversee, direct and manage Supportive Services Department including Licensed Nursing Assistants and Registered Nurses.
  • Coordinate the Quality Assurance Program and attend the Advisory Committee.
  • Provide services as required in the RN job description.
  • Stipulate the Health Home Care Provider (HHCP) retains professional responsibility for all care and services provided.
