Lakes Region Mental Health Center (LRMHC) is the community mental health center serving Belknap and southern Grafton Counties. We provide services to 24 surrounding towns, and nearly 4,000 patients each year. A non-profit, private corporation founded in 1966, Lakes Region Mental Health Center is governed by a volunteer Board of Directors dedicated to our mission.  We provide integrated mental and physical health care to children, adults, elders and their families who are living with – and recovering from- mental illness and/or emotional distress. Our dedicated professionals provide the highest quality of healthcare and collaborate with many other organizations in the region to improve access to care and meet the mental health needs of the community. DESCRIPTION OF DUTIES & TASKS:

Under the direction of the CSP Assistant Director, and within the agency’s program policies and procedures, the SE Facilitator performs the following functions:  Provides direct individual services for patients in the Supported Employment Program with a rehabilitative focus.   Interfaces with the community and within the agency to ensure optimum quality and comprehensiveness of services.  Oversees staff in the Supported Employment Program and provides overall program development and oversight.

 

ESSENTIAL FUNCTIONS:

  1. Provides vocational counseling and rehabilitative services to patients within the Community Support Program (CSP).
  2. Provides on-site support for patients on supported employment and offers direct vocational counseling and intervention for patients around vocational planning, job seeking and retention, etc., as it relates to their individual treatment plan.   May be required to transport patients in their personal vehicle.
  3. Completes required documentation in a timely and thorough manner according to established standards.  Maintains standards of direct service hours as established for the position.
  4. Investigates and follows up on employment and training opportunities for patients and maintains records of vocational resources and shares these with patients/team as appropriate.
  5. Assists patients to maintain community living by teaching activities of daily living and personal care; promotes independent and productive habits to patients in the community. Also, develops other programs and activities that promote the introduction and integration of the client into the community.
  6. Participates actively in staffing and seeks out supervision on a regular and as needed basis; makes contributions to staff meetings and applies what they have learned effectively.  Establishes staff development plan with supervisor and makes concerted effort to seek training in relevant areas.
  7. Informs supervisor and others as necessary of schedule, activities planned, and outcomes to maintain continuity, and prioritizes these in supervision to prevent burnout or misallocation of time.
  8. Provides community education informally as appropriate and conducts formal education presentations as required.  Represents the client/program/agency positively in interfaces within and outside of the agency.   Assists Assistant Director in general program planning and vocational development for patients.
  9. Assists in the development and preparation of data and reports.
  10. Supervises staff and may participate in hiring of staff and provide orientation and training of staff.
  11. Maintains necessary credentialing to perform job duties.
  12. Assists with Quality Improvement and Utilization Review functions.
  13. Participates in program marketing and public relations efforts and provides community education and consultation.
  14. Serves on agency committees as requested.
  15. Monitors outcome measures for patient recovery.
  16. Performs other work-related duties as assigned by their Supervisor.
  17. Attendance is an essential function of this position.

 

REQUIRED KNOWLEDGE, SKILLS & ABILITIES:

The individual must possess the knowledge, skills and ability noted below or be able to explain and demonstrate how the individual can perform the essential functions of the job, with or without reasonable accommodation, using some combination of skills and abilities.  Must also posses the necessary physical capacities, with or without the aid of mechanical devices or other forms of accommodation, to safely perform the primary functions of the job.

  1. Working knowledge of diagnosis, treatment planning, medications, counseling and crisis intervention techniques.
  2. Must be able to establish a rapport with the patients.  Must treat all patients with dignity and respect.
  3. Ability to organize work-load so that his/her clinical and administrative duties are completed in a timely and thorough manner without the need for direct supervision.
  4. Understands the LRMHC mission and applies this knowledge productively to shape internal policies and external contacts.
  5. Must be capable of working closely with other agency staff to expand the knowledge of mental health services both within the Child and Family Team and across to other programs.  Must be able to develop strong working relationships with external services.
  6. Must have basic computer skills, and strong verbal and written communication skills.
  7. Must have the ability to read and analyze data and reports.
  8. Must be able to tolerate periods of heavy workload, to include long working hours and the balancing of conflicting demands
  9. Attendance is a requirement of this position.

 

QUALIFICATIONS:

Education:

Bachelor or Master’s Degree in a Human Services area with emphasis in mental health and/or vocational rehabilitation.
