Access to Care Therapist

Lakes Region Mental Health Center

Lakes Region Mental Health Center (LRMHC) is the community mental health center serving Belknap and southern Grafton Counties. We provide services to 24 surrounding towns, and nearly 4,000 patients each year. A non-profit, private corporation founded in 1966, Lakes Region Mental Health Center is governed by a volunteer Board of Directors dedicated to our mission.  We provide integrated mental and physical health care to children, adults, elders and their families who are living with – and recovering from- mental illness and/or emotional distress. Our dedicated professionals provide the highest quality of healthcare and collaborate with many other organizations in the region to improve access to care and meet the mental health needs of the community. By working in partnership with other health care providers, hospitals, law enforcement, corrections, courts, school districts, social workers and public health officials, we are able to provide a continuum of care for our patients and improve the quality of life in the Lakes Region.


Works with agency Access to Care Team to provide same day assessments to patients (70%).  Provides supervision and mentoring for clinical staff and clinical interns, provides other administrative oversight (30%) and acts as a liaison for the Access to Care Team to other areas of the agency and community.

Requirements:

The individual must possess the knowledge, skills and ability noted below, or be able to explain and demonstrate how they can perform the essential functions of the job, with or without reasonable accommodation, using some combination of skills and abilities.  Must also posses the necessary physical capacities, with or without the aid of mechanical devices or other forms of accommodation, to safely perform the primary functions of the job.

•Is able to establish a rapport with patients.  Treats patients with dignity and respect and takes a patient centered approach to treatment, as noted through supervision, team participation, and feedback from patients and staff.  Is able to build a relationship of trust, as observed by patients, supervisor, and team.
•Has general knowledge about medications and diagnosis as well as a working knowledge of issues, rehabilitation and supportive living concepts associated with patients and uses these appropriately with patients.
•Has working knowledge of crisis intervention techniques and counseling principles and remains current in EBP’s provided to patients.
•With the assistance of the supervisor, is able to organize and prioritize a workload so that rehabilitative and administrative duties are completed on time and in accordance with performance standards.
•Follows through with responsibilities to the satisfaction of contact persons and/or supervisor.
•Learns the philosophy and operating procedures of LRMH and applies them effectively both within and outside the agency.
•Represents the patients/program/agency constructively both within and outside the agency, serving on agency and community committees as appropriate.
•Maintains general knowledge of community resources and presents these to patients/supervisor/team as appropriate.
•Maintains a working knowledge of the benefits systems and utilizes this information to the best advantage of the patients.
•Ability to use computers and word processing equipment.
•Ability to express or exchange ideas by means of the spoken word and to receive detailed information through oral communication.  Primary functions include activities in which verbal communication with patients and with others is key to job effectiveness.
•Visual acuity must meet a minimum standard to work with preparing, reading and analyzing data and reports, work with computer terminals, etc.

Qualifications:

Education:  Master’s degree in counseling, social work, or other human service field.

Experience: 1-2 years of experience with emphasis in mental health. Must maintain personal auto insurance at a level commensurate with agency requirements

Licenses/Certifications:  Must be licensed in NH by the Board of Mental Health Practice. Must possess a valid driver’s license

Responsibilities:

Under the general supervision of the Director and within the agencies and program’s policies and procedures, the Therapist III performs the following functions:

Essential Functions:
1.Develops an effective therapeutic relationship with a diverse group of patients.
2.Provides intake assessments for Child and Family Services, Community Support Services, Acute and Short Term Services and Substance Use Disorder Services.
3.Participates actively in supervision, team meetings, ISP planning; makes contributions and applies what is learned effectively.
4.Manages a variety of tasks and demands.  Prioritizes competing needs in a way that is most helpful to patients and is an efficient use of time.  Participates in team activities and promotes supports for program initiatives. 
5.Establishes a staff development plan with supervisor and makes a concerted effort to seek training into areas identified in the plan.  Provides schedule, activities planned and outcomes and priorities within EMR and to supervisor.
6.Provides community education informally as appropriate and participates in more formal educational presentations as requested.
7.Maintains standards of patient access hours as established for the position.  Completes all required documentation in a timely fashion
8.Understands the mission and core values of Lakes Region Mental Health Center and represents the agency positively within and outside the agency. 
9.Participates in agency committees and in program performance reviews and audits. 
10.Maintains credentials necessary to provide supervision towards licensure for Master’s level staff and coordinates and provides supervision for Bachelor Level and Master’s Level interns. 
11.Attendance is a job requirement.
12.Performs other duties as assigned.
