Catholic Charities New Hampshire is looking for a part-time Intake Coordinator/Navigator (21 hrs/wk) to join our team of dedicated professionals at our Lebanon Service Location.

Requirements:
  • Excellent interpersonal and communication skills
  • Second language proficiency or fluency a plus
  • Can demonstrate compassion and empathy in working with Clients of all socio-economic backgrounds
  • Commitment to Clients’ welfare
  • Basic bookkeeping skills
  • Intermediate level skills in MS Office software programs: Word, Excel, outlook email
  • Computer aptitude and ability to work with database software programs to record client interactions with ease
  • General understanding of social services practices & resources
  • Thorough understanding of, and ability to follow, standard operating procedures
  • Calm, under pressure; patient and understanding with both Clients and colleagues
  • Maturity and common sense
  • Integrity, moral character, ability to keep confidentiality as appropriate
  • Always presents self as positive ambassador/representative of CCNH
  • Team player: willing and able to effectively coordinate actions with other CCNH Staff and participate in CCNH fund-raising activities if necessary
  • Excellent judge of character

Education

  • Bachelor’s Degree preferably in Social Work, Psychology, Anthropology, Sociology or similar field of study
Responsibilities:
  • Act as the first or second point of contact for all individuals who call or walk into our offices seeking assistance, providing basic information and referral service, helping Clients complete the Intake process, and forwarding information to appropriate colleagues via entries in Apricot and other software systems
  • Help clients clarify their needs and understand their options and refer to appropriate internal and/or external resources (triage)
  • Manage distribution of Emergency Assistance funds under the supervision of senior staff
  • Provide intensive case management services to clients who are willing and able to engage in a deep working relationship with us.  The goal is to help those clients move their lives forward in a meaningful and sustainable way
  • Work with clients to establish Life Plan Goals that are appropriate to their situation and capabilities including meeting with clients frequently to check progress, provide financial fitness and life skills training, coaching, support and encouragement as necessary
