Spiritual Care Counselor

Current Opening Monday - Friday 8 a.m. - 4 p.m.

Reporting to the Hospice Care Services Manager, the Hospice Spiritual Care Counselor is responsible for providing spiritual care for agency patients and families, in particular for Hospice patients.

 

Essential Functions

  • Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. 
  • Work effectively with members of the Hospice team in establishing, implementing, and evaluating the care plan for each family regarding spiritual and bereavement needs.
  • Provides spiritual counseling directly to patients and families according to assessment of needs in a manner consistent with patient and family beliefs and desires.
  • Communicates with local clergy/pastoral counselors to support patients and families spiritual needs.
  • Completes spiritual assessments and visit information and outcomes according to procedure and professional standards.
  • Participate in, facilitate or develop in-service education for Program and Agency staff as required by the manager.
  • Participate in the development and ongoing evaluation of the spiritual components of the care plan.

 

Other Job Functions

  • Participate in interdisciplinary group meetings and other case conferences as requested.
  • Actively participate in team/staff meetings and provide constructive input to the meeting leader.
  • Participate in planning for the agency memorial services, patient's memorial services, and facility memorial services as requested and approved by manager.
  • Assist in supervision and communication with Spiritual Care Volunteers and students when involved with a patient on your caseload.
  • Identify problems and offer constructive suggestions/solutions. Accept and follow through with final decisions.
  • Work collaboratively and in a professional manner with the management team and staff from other areas.
  • Demonstrate flexibility in role by accepting other duties as assigned for which qualified.
Requirements:

Minimum Qualifications

  • Master's Degree in Divinity or related degree and at least 2 units of CPE (Clinical Pastoral Education)
  • Board certified or eligible preferred

 

Physical Demands
This position requires the ability to talk and hear; read, write and speak in English, normal or correctable vision, and hearing.
Intermittent physical activity, including standing, walking, bending, kneeling, stooping and crouching as well as lifting and supporting patients.  Ability to minimally lift up to 25lbs. and the ability to provide up to 75% assistance for patient transfers.
This position requires the ability to frequently enter and exit and/or sit in an automobile for prolonged periods while commuting to work assignments.

 

Work Environment

The employee will be exposed to variety of uncontrolled work environments. The employee may be exposed to all kinds of weather conditions while driving to and from work assignments.  The employee may also be exposed to blood and bodily fluids, virus, disease and infection from patients and environmental hazards in the work environment.  The employee may be required to work extended periods of time using an electronic device.
