Case Manager

Riverbend Community Mental Health

Core services include 24/7 emergency mobile crisis services; counseling services for children, adults, and families; addiction treatment; and community support, vocational services, and residential programs for adults and seniors. Case Manager - Crisis Treatment Center

Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking part-time Case Managers for our new Crisis Treatment Center.  The Case Manager works with clients (and their families and caregivers) who are experiencing a behavioral health crisis as a member of the treatment team.  Case Managers assist with promoting timely access to appropriate levels of community-based care.  We have shifts available on Saturday and Sunday, 8AM-5PM, and as needed during the week.    

Responsibilities

  • Serve as the contact point, advocate, and informational resource for clients, the multidisciplinary treatment team, family/caregiver(s), and community resources.
  • Supports clients access to programs/resources needed to maintain community tenure.
  • Work with clients to plan and monitor care.
  • Assess patients’ unmet health and social needs and work to link client with appropriate resources.
  • Develop and support the implementation of a community care plan (emergency plan, health management plan, and ongoing action plan) with the patient, family/caregiver(s) and providers, as appropriate. 
  • Monitor adherence to care plans, evaluate effectiveness, monitor patient progress in a timely manner, and facilitate changes as needed.
  • Educate client and family/caregiver(s) about relevant community resources.
  • Attend meetings between client, family/caregiver(s), and care team.

Requirements:
  • Bachelor’s or Master’s degree in Psychology, Social Work, Education, Rehabilitation, or Human Services is required.
  • Experience providing case management or care coordination services in an organization providing behavioral health, substance use disorder, or emergency room services is preferred.
  • Knowledge of crisis intervention and Illness Management and Recovery (IMR) is required.
  • Valid driver’s license, personal vehicle, and auto insurance or access to a reasonable alternative required.
  • Excellent communication, problem-solving, time management, and organizational skills are required.
