Housing Stabilization


The Housing Stabilization Coordinator will work with local behavioral health providers to identify individuals or households that are Medicaid recipients that are having difficulties identifying, procuring, and/or sustaining decent, safe, and affordable housing. The emphasis of this position is on the importance of housing as a primary social determinant of health.

Requirements:

• Strong computer skills including Microsoft Word & Excel with the ability to learn new programs, as needed
• Ability to be flexible
• Organizational and prioritization skills
• Ability to solve problems, make decisions, and resolve conflicts
• Experience in Human Services
• High School Diploma or GED
• Customer Service Experience
• Valid NH Driver’s license.
• Proven ability in written and oral communication

Responsibilities:

• Work with local behavioral health providers to identify individuals or households in need of housing stabilization services and meet with them to assess their support service needs.
• Record whether initial assistance is for identification/procurement of safe and affordable housing or for the sustaining of safe and affordable housing.
• Provide individualized client support to address any barriers to housing.
• Assist clients with procuring necessary documents such as identification card, birth certificate, certification of homelessness, and other documents as needed.
• Track each household to document direct services provided as well as additional resource and referral.
• Develop a working relationship with area landlords to maximize housing opportunities within Sullivan County.
• Maintain an updated list of area landlords, properties, and rental information to maximize housing opportunities within Sullivan County.
• Attend Sullivan County Partner Meetings on a monthly basis.
• Attend Complex Care Team Meetings on a monthly basis.
• Assist with coordination of and securing attendance for the SCS educational workshops in Sullivan County (Life Skills, Tenancy 101, and Financial Literacy).
• Make appropriate referrals to resources outside the scope of Housing Stabilization Services when appropriate.
• Generate client data for comprehensive weekly and monthly reports, including outcomes, successes, and challenges and submit to the Administrative & Stabilization Manager
