TLP Care Coordinator/Case Manager

TLP Care Coordinator/Case Manager: Transitional Living Program in Manchester, NH is seeking a care coordinator to provide services to youth ages 18-22 that are experiencing homelessness or at risk of homelessness using a Positive Youth Development approach. This position includes outreach and engagement of at-risk youth prior to program enrollment, intensive case management and care coordination during a youth's stay and on-going after-care once the young person transitions from the home. The Care Coordinator will work with a maximum of 9 youth residents at a time to achieve self-sufficiency upon exit including a focus on education, job readiness, and life skills building both individually and in groups. Duties also include the recruitment, training and supervision of the 2 Resident Assistants who serve as volunteer mentors, guides, and oversight in the home. Some oversight of building maintenance is also required. Requirements:

 BA + 3 years related experience is required, particularly as it relates to disconnected and vulnerable youth. A working knowledge of drug and or alcohol use, mental health, and knowledge of community resources is a plus. You must be able to work independently and work flexible hours. The ideal candidate will be passionate, self motivated, energetic, enthusiastic, and have a heart for working with young adults. The chosen candidate will also be required to submit to a criminal background and motor vehicle record check. Candidate must have a valid driver’s license and auto insurance that meets company standards. The ability to travel throughout the Manchester area and beyond is necessary and at times.
