Director of Integrated Care

Greater Nashua Mental Health Center

Established in 1920, Greater Nashua Mental Health Center (GNMHC) has as its mission: empowering individuals to lead full and satisfying lives through treatment and support. Our agency provides comprehensive, evidence-based behavioral health services for individuals with mental health and/or substance misuse disorders.  We offer a wide array of services in office and community settings, serving adults and children.  As the State-designated Community Mental Health Center for Southern Hillsborough County, we serve everyone who walks in our doors, and no one is ever turned away due to the inability to pay for services.  Without our agency, many of the most vulnerable members of our community would not have access to critically needed behavioral health care.   We believe that by respecting people as they are, we can help them to become all that they wish to be.

The Director of Integrated Care will lead and manage behavioral health integration projects at Greater Nashua Mental Health Center including ProHealth NH and InteGreat Health. These programs involve close collaboration and integration of our community mental health center and a local Federally Qualified Health Center (FQHC). Program goals include increasing patients’ access to integrated primary and behavioral health care, reducing gaps in care during transitions and supporting the delivery of integrated care.

The Director of Integrated Care is responsible for implementation of plans to move projects from co-located to integrated care teams within the SAMHSA Integrated Care model framework ensuring a holistic, whole-person approach is taken towards providing a health home for clients. The Director will supervise GNMHC’s Integrated Care Case Manager, work closely with other staff of the FQHC, and ensure processes meet or exceed regulatory and industry standards and meet requirements of our contractual relationships with the State of New Hampshire, Integrated Delivery Network, and other entities.

The Director will also develop and monitor essential activities of integrated care, document significant events, and provide analysis of reported data for integrated care projects. Additional responsibilities include the promotion and education of programs to stakeholders and working with a diverse client population in a variety of approaches and/or settings.

Qualifications include a minimum of 1 year supervisory level experience with team based care, integrated primary care, or population health management, an understanding of the SAMHSA Integrated Care model framework and use of Community Health Workers, proven ability to develop and monitor workflows and budgets, and experience working with physicians and other healthcare related stakeholders. A Masters or Doctoral Degree in nursing, social work, psychology, health administration or a related human services field is required. Ideal candidate will have excellent interpersonal and communication skills, organizational skills, and experience coordinating multiple projects.
