Residential Case Manager

Dover Children’s Home, founded in 1893, is a non-profit intermediate-level treatment program, offering a wide array of services to children in need from all parts of NH.  The agency is designed to provide community-based residential care for boys and girls who have been abused or neglected; are under a CHINS (Child in Need of Services) petition; or who have committed delinquent acts.  DCH is licensed to provide treatment for up to 19 children, ages 12 - 18.  The age is extended to age 20 in our independent living program (the PILOT House). In May 2015, the agency was additionally licensed to provide residential treatment through our Pregnant and Young Parent Support Program to adolescents who are currently pregnant and/or parenting, as well as their young children.

Position Description:

This is a direct-care position.  Case Manager is responsible for the oversight of daily activities, needs and safety of all residents.  Case Manager is responsible for assessing the needs and implementing the agreed upon treatment goals for all residents.  The position requires active participation in treatment planning for residents assigned to the CM.  CM is also responsible for meeting with assigned residents to discuss progress toward treatment plan goals, and is responsible for the completion of all assigned documentation for their assigned residents.  This position reports directly to the assigned Residential Supervisor.

 

Responsibilities:

  • Work assigned schedule of 40 hours per week, reporting to work promptly as scheduled.
  • Know and adhere to agency policies and procedures.
  • Maintain positive working relationships with staff and residents.
  • Maintain positive and professional working relationships with the general public, placement agencies, families and community members.
  • Demonstrate an enthusiastic commitment to the job and agency.
  • Accept and integrate supervision and feedback.
  • Maintain up-to-date compliance trainings and provide all necessary personnel file documentation to the Program Director in a timely manner.

 

Specific Duties:

  • Assist assigned resident(s) with learning the program rules and expectations and reviewing preliminary treatment plan goals.
  • Facilitate 1:1 weekly facetime with assigned resident(s) to build relationships and discuss progress toward treatment plan goals, as well as other needs and concerns.
  • Establish a professional relationship with resident’s treatment team members and family when appropriate.
  • Participate in ongoing clinical consultation as needed with the Clinical Director, Program Director or Executive Director to discuss interventions, patterns of behavior and/or family needs and concerns.
  • Complete Monthly Reports, during assigned shifts, for all assigned residents and provide to the Clinical Director by the 7th of the following month.
  • Ensure all necessary resident documentation is saved on the computer in order for staff to access as needed.
  • Utilize staff meeting case review documentation to gather information to complete monthlies with accurate information.
  • Collect ongoing photographs and complete a “Good-Bye” Photo Album for each resident to provide as a gift at the time of discharge.
  • Gather all appropriate discharge documentation and provide to the Clinical Director during the week of the resident’s discharge.
  • Plan birthday and special occasion celebrations for assigned residents (i.e. request necessary gift cards, funds from the appropriate sources).
  • Oversee the dispensing of all medication and documentation of medication for each resident.
  • Oversee all daily living activities for each resident.
  • Adhere to the daily schedule of the program, and shift plan schedule as assigned by Shift Supervisor.
  • Provide safe transportation to meet the needs of all residents.
  • Complete all necessary incident reports by the end of shift with factual, objective content and language.
  • All other tasks as assigned by Shift Supervisor, Program Director, Clinical Director.
Requirements:

Bachelors degree in related field. Experience working with teens and or similar line of work preferred.
