 The Community Health Access Network (CHAN) is the only Health Center Controlled Network (HCCN) in New Hampshire.  Established in 1995, CHAN has developed an integrated clinical and administrative system infrastructure for its five Federally Qualified Health Center (FQHC) Full Members and its four Affiliate Members (two FQHC’s, two Healthcare for the Homeless FQHC’s).  CHAN's vision is to provide high quality, leading edge clinical systems, using national guidelines, to providers of the medically underserved in an efficient and transparent manner, thereby supporting the use of best practices to minimize the cost of health care for all. 

For more than ten years, CHAN has methodically expanded a state-of-the-art infrastructure for technology suported health care delivery.  Central to CHAN's focus has been the automation of the primary care record.  CHAN's systems include a fully integrated Electronic Medical Record (EMR) (GE Centricity) which support 90 providers and their patients.  The EMR is linked to the GE Centricity Practice Management (PM) System and shares a common reporting tool.  The EMR also has created links to member reference labs and four hospitals. 

CHAN's current infrastructure, the commitment and experience of our staff and members and our demonstrated experience in effectively impelmenting complex HIT projects makes CHAN extremely well-positioned to offer consultation in numerous areas including Electronic Health Record and Practice Management implementation, work flow (re)design, electronic form design, clinical reporting, health information exchange and more. 

CHAN is a 2008 recipient of the Healthcare Information and Management Systems Society (HIMSS) Davies Award in the Community Health Organization (CHO) category.  Since 1994, the HIMSS Nicholas E. Davies Awards of Excellence Program honors achievement in the implementation and use of health information technology, specifically electronic health records, for healthcare organizations, private practices, public health entities, and community health organizations.
