Description:
A Physical Therapist needed to deliver skilled care to patients in their homes.
We are seeking a Physicial Therapist that is:
- Caring
- Compassionate
- Detail oriented
- Respectful
- Dedicated
- Can work independently and as part of a healthcare team to meet patient needs.
- Able to understand and carry out directions and instructions, record messages and keep records.
- Able to read, write and speak English.
- Physically and emotionally capable of performing the job requirements.
Requirements:
Travel, weekend and holiday rotations are required for this position.
We are looking for a PT with:
- Current New Hampshire licensure.
- Bachelor's or Master's Degree in Physical Therapy from a program approved by the American Physical Therapy Association.
- Preferably 2 years experience in an inpatient and/or community health setting.
Responsibilities:
- Provide initial patient admission and ongoing patient evaluations and treatment.
- Assess muscle, nerve, joint, skin and functional ability as appropriate.
- Develop and implement plan of care, recommend treatment intervention in accordance with physician's orders.
- Direct and aid patients in active and passive exercises, muscle re-education, gait and functional training.
- Provide instructions to patient in various exercise programs and therapeutic procedures for home-care therapy.
- Supervision of Physical Therapy Assistants and manging a caseload of patients.
