Integrated Care Clinician

Core services include 24/7 emergency mobile crisis services; counseling services for children, adults, and families; and community support, vocational services, and residential programs for adults and seniors. Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a full-time clinician to work in our growing integrated care initiative through Riverbend Counseling Associates.

As an Integrated Care Clinician, you’ll be joining a fantastic team with experience and expertise in a variety of treatment modalities and will be embedded in a Concord Area primary care office associated with Concord Hospital or Dartmouth Hitchcock.   The emerging system of integrating behavioral and physical care has numerous opportunities for learning and collaboration.

  • Master’s degree in Psychology, Social Work, or a related Human Services field is required.
  • NH Licensure as LCMHC, LMFT, LICSW, or Psychologist is required.
  • Experience in dual diagnosis interventions preferred.
  • Clinical leadership skills are a plus.
  • Availability Monday through Friday during traditional business hours is required.

NOTE: If you have credentials from another state, our Human Resources Department can assist the successful candidate in securing the appropriate licensure to practice in NH

  • Provide therapeutic evaluations and interventions utilizing a variety of short-term treatment approaches, working with individuals, couples, families, and groups.
  • Responsible for coordination of care with and/or referrals to other behavioral health services as appropriate.
  • Employ short-term, evidence-based treatment modalities with your clients as appropriate.
  • Provide education and consultation to the practice physicians and staff around management of patients’ psychosocial and care coordination issues. 
  • Provide care management for patients in the medical practice with psychosocial and/or complex health needs with a goal of supporting care team in helping patients follow treatment plans, assessing patient needs for behavioral health support and treatment, providing short-term therapy and referrals as needed, and preventing Emergency Department use and hospitalization. 
  • Participate in a multi-disciplinary team environment.
