Personal Mentors

PathWays of the River Valley

PathWays is an Area Agency designated by the State of NH to be the single point of entry to community based supports for individuals with developmental disabilities and their families in Sullivan and lower Grafton counties in NH. 

Our misson:  PathWays is dedicated to expanding the opportunities that enrich the lives of people with disabilities.  Our passion is working together with children, adults and their families to create their own desired future in their own community.  We provide our services in a spirit of partnership and respect.

Services we provide:

Family Centered Early Supports and Services.  Children aged birth to three, who are at risk of developmental delay, often with significant medical issues receive home-based services with a therapeutic focus with goal of minimizing permanent effects of delay. 

Family Support. The family is at the center of this service and defines both their needs and the type of engagement they have with the agency. Services provided include respite, educational advocacy, information and referral, transition from school to adult services and person-centered planning. 

In-Home Supports. PathWays provides in-home supports to children and adults who continue to live with their families. These services may focus on personal care, skill acquisition and family defined supports. These supports are tailored to each family with the common goal of keeping the family together.

Service Coordination. This service provides a central person to make sure that an individual has all the services and supports they need in all aspects of their life. This includes engaging in the development of individualized service plans and the monitoring of community-based services.

Outreach. Limited support is provided to facilitate the independent living of individuals in the community through support with money management, grocery shopping, or other life skills.

Vocational Supports. The focus of these supports is identifying employment opportunities, getting a job and providing support to maintain it. This includes working with local businesses to develop work opportunities for individuals with disabilities.

Day Services. This is a broad range of services provided to individuals in their local communities. This may include recreational opportunities, volunteer activities, work experiences, relationship building and personal skill development.

Residential Supports. This is a wide range of services, which may include in-home supports to keep families together, adult foster care, individualized supports and small group arrangements. These supports may range in intensity for several hours a day to 24-hour care and may involve specialized medical or behavioral supports.


Personal Mentors: Part time. Works 1:1 with individuals with development disabilities to provide guidance and support as needed in the areas of safety, daily living, communication, community, social and leisure skills. Supports individuals to participate in activities and access resources that will assist them in achieving their personal goals. Some service provision may occur in the individual’s home or at a worksite. Opportunities are throughout lower Grafton and Sullivan counties (see list below). Good verbal and written English communication skills as well as basic math and computer skills (MSOffice) required. Interested in assisting individuals achieve personal outcomes. Must be able and willing to work a flexible schedule and at varied locations as needed.

Lebanon

  • 8 hrs/wk, $13/hr
  • 5 hrs/wk, 8-4 Sat & Sun, $14/hr
  • Temporary position, 24 hrs/week, (2) 12 hour shifts, 8pm-8am, $15/hour
  • 16 hrs/wk, M, T, W afternoon shifts, Fri and Sat full days or split shifts, flexible hrs but within med admin times
  • 30 hrs/wk, 6 days/week, hrs within 2pm and 8pm, $14/hr
  • 10 hrs/wk, two 5 hour days/week, $13/hr
  • 30 hrs/wk, Saturday and Sunday and two evenings a week, $15.00/hr

Enfield

  • 20 hrs/week, Monday - Thursday, 9 am - 2 pm; $15/hour

Acworth/Charlestown

  • 30 hrs/wk, $15/hr #1045

Washington/Newport

  • 40 hours/wk, Most days starting at 8 am, some evenings and occasional weekends, $14/hr

Newport

  • 21 hours/wk, Monday 8:30 am – 1:30 pm, Tues – Fri 10 am – 2 pm, $13.90/hr

Claremont

  • 8 hours/wk, 8am – 12pm Tues and Thurs, $11.30
  • 25 hrs/wk, Mon-Fri daytime schedule, $14.50/hr

Charlestown

  • 8 hrs/wk, M-Th, 2-5, $13.50/hr
  • 20 hrs/wk, Mon-Fri daytime schedule, $13.50/hr

Plainfield

  • 28 hrs/wk, 4 or 5 days, M-F, $13/hr
