Clinical Case Manager - Adults

Riverbend Community Mental Health, Inc., one of the largest providers of community-based mental health services in New Hampshire, is seeking a full-time Adult Clinical Case Manager to join our talented multi-disciplinary team in our Franklin office working with adults with serious and persistent mental illness.
 
As a Riverbend Clinical Case Manager, your primary responsibilities will be to provide clinical consultation, case management and therapy services to adults living with mental illness using best practices and a number of evidence-based models. This is a great opportunity to work on the front lines of community-based mental health services, and help our clients achieve optimal recovery and independence.
 
Responsibilities
·         Individual and group therapy both in in the office an in the community
·         Case management and interagency coordination
·         Crisis management
·         Treatment planning
·         Clinical Documentation in accordance with medical necessity guidelines and agency standards
·         Same Day Assessment availability

Requirements:

·         Master’s degree in social work, psychology, or related field
·         Degree with eligibility for NH License as LCMHC, LICSW or LMFT is required
·         Valid driver’s license, personal vehicle, and auto insurance
 
Note: If you have credentials from another state, our Human Resources Department can assist the successful candidate in securing the appropriate licensure to practice in NH
