Case Manager

Riverbend Community Mental Health

Core services include 24/7 emergency mobile crisis services; counseling services for children, adults, and families; and community support, vocational services, and residential programs for adults and seniors. Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a full-time Case Manager to join our talented multi-disciplinary team in our Community Support Program working with adults living with severe and persistent mental illness.

As a Riverbend Case Manager, you'll work closely with clients to coordinate their treatment, by assisting with obtaining the services they need, and facilitating communication with the treatment team within Riverbend and at outside agencies.

 
Essential Duties and Responsibilities: 
 

  • Help clients and family understand treatment options and make informed decisions
  • Work with treatment team to facilitate case coordination and information sharing
  • Participate in the development and implementation of client treatment goals
  • Work with clients to build symptom management skills
  • Promote effective utilization of clinical resources and other social supports
Requirements:

Education and/or Experience Requirements:
 

  • Bachelor’s degree in Psychology, Social Work, Education, Rehabilitation, or Human Services 
  • Valid driver’s license, personal vehicle, and auto insurance or access to a reasonable alternative.
  • Excellent communication, problem-solving, time management, and organizational skills.
