Harbor Homes is a non-profit community-benefit organization that provides low-income, homeless, and disabled New Hampshire community members with affordable housing, primary and behavioral health care, employment and job training, and supportive services. As a member of the Partnership for Successful Living, Harbor Homes offers a unique, holistic approach to care that results in better outcomes for our clients and the community.

As a Patient Navigator, you will support and facilitate outreach and enrollment activities to eligible Health Center patients and other consumers that will enable them to obtain affordable health insurance through the Health Insurance Marketplace.  You will be responsible for partnering with local community agencies such as shelters; soup kitchens; Community Action Program; local welfare, employment and substance abuse agencies; the VA; police; library; housing and other agencies and individuals which provide services to low income and homeless individuals.  Additional duties will include but are not limited to access and triage for services provided in the Harbor Care Health and Wellness Center and assisting in the preparation of annual grant requests. 

Bachelor’s Degree required with 1 to 3 years of relevant experience.  Strong knowledge of health care environment, health care market place and homeless community and culture along with a valid driver’s license also needed.  Must have excellent communication skills and ability to work independently in a fast-paced environment juggling many priorities.
